Regularly contact or communicate with patients regarding current medication utilizations and ongoing needs, communicating regularly with in-house or contract pharmacies. Complete routine reassessment of patients' medication utilization and identify concerns or side effects patients may be experiencing. Consult with pharmacists or providers regarding all clinical needs of patients, routing all clinical interventions or counseling to a pharmacist or appropriate licensed clinician to complete. Assist patients with scheduling prescriptions, fills, or deliveries. Document all patient communication in specialty pharmacy software. Generate reports routinely to identify new patients, upcoming medication needs, and adherence-related data. Room patients who attend visits in person, collecting vitals, and updating labs, pharmacy, and other pertinent health information. At the end of in person visits, ensure that patients obtain labs while onsite, schedule follow up appointments, and have printed orders for other tests such as chest x-rays or bone scans. Complete health questionnaires with patients when needed onsite, leveraging tablets and other technologies. Assist patients with installing and logging into the AndHealth app on their mobile device. Position Summary This is a full-time (4x10 hour shift/week) inpatient pharmacist position. The scheduled shift times may vary. Responsibilities * Monitor patient medications to determine possible medication therapy problems. * Serve as Interventional Pharmacist competently per Pharmacy Practice Guidelines. * Supervise and/or prepare intravenous admixture solutions according to physician orders. * Check work of technical staff for accuracy. * Understand emergency drugs and thrombolytic therapy (including continuous infusion, normal dilution, dose and monitoring during infusion). * Respond to Code Blues, Traumas, STEMI's, and Rapid Sequence Intubations in the ED and throughout the hospital. Prepare prescribed medications, including compounded formulations and IV's * Evaluate cultures and sensitivities and make antimicrobial recommendations to physicians as appropriate. * Provide drug information to physicians, nurses, and patients. * Supervise ED Certified Pharmacy Technicians by utilizing good team-building skills, in order to unite ED Pharmacy staff around common goals and generating greater productivity. Requirements * Pharm D or Bachelor's degree in pharmacy. * 3-5 years institutional experience preferred, but not required. * Licensure as a Pharmacist in the State of Ohio. * Analytic ability to determine drug appropriateness and evaluate drug instructions. * Ability to provide care for a neo-natal, pediatric, adolescent, adult and geriatric patient population. * PALS and ACLS Certification preferred (opportunity to obtain this post-employment will be provided if needed). * LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
