The PhysicalTherapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in an independent living facility. Powerback Rehabilitation To You allows the PhysicalTherapist to evaluate and treat in the most functional environment which is the patient's home. The PhysicalTherapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The PhysicalTherapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. They implement individualized therapy programs designed to restore, reinforce, and enhance performance. In addition, the PhysicalTherapist instructs, educates and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. They direct activities to promote and maintain health. This position provides a flexible schedule and work environment in various settings.
1. As necessary or applicable, supervision of Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.2. Responsible for adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings.3. Attends and contributes to patient care, staffing conferences and other related meetings.4. Makes referrals to other disciplines and agencies in order to address the comprehensive needs of the patient.5. Consults with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.6. Adapts program and/or plan of care according to the needs of the individual patient.7. Promotes a safe environment and enforces the elimination of fire and safety hazards.8. Orders supplies and equipment as necessary.9. Cleans, maintains, identifies, and reports the need for repair of equipment as necessary.10. Puts patient service first: ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.11.Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the physical therapy treatment plan, and completes all related documentation and record keeping regarding these services. Services include but are not limited to application of therapeutic techniques in areas such as:* Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training

* Use of electrotherapeutic, physical agent and mechanical modalities

* Case Management in the home and community12. Designs, fabricates, fits and/or applies adaptive devices, and trains patients in the use of adaptive devices included orthotics and prosthetics.13. Selects appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest.14. Instructs patient in the skills and techniques required for participation in mobility activities and evaluates patient progress, attitudes, and behavior as related to rehabilitative potential.15. Performs routine reexamination as needed/required to modify/progress plan of treatment.16. Incorporates health and wellness programming into treatment plans for patients.17. Performs home and community mobility assessments and provides recommendations for safe mobility in the home and community space.18. Performs other related duties as required. Qualifications: 1. Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility; and 2. They must have a Master's degree in Physical Therapy; or 3. They must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or 4. They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience. 5. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy. 6. The Role
The Addiction Counselor is an integral member of the clinical team, responsible for managing a caseload of patients with substance use disorders. Collects and reviews referral information upon assignment of patients from the referral source and consults with others as needed to support coordination of services and scheduling of patient interviews.
Based on assessments, make referrals to the most appropriate programs and agencies in an effort to help patients access the right level of care to best meet their treatment needs.
Assists patients with scheduling an admission or first appointment date at the program or facility where they are referred.
Monitors patient access to and involvement with treatment and other providers to help address barriers to treatment and the recovery process.

The Pines at Shrub Oak is a 24-bed, coeducational, interim residential placement for students on the autism spectrum who may need significant psychiatric support and stabilization before transitioning to a less restrictive environment. Students at the Pines receive psychiatric services in addition to the academic and life skills instruction in a safe, supportive environment with a 2:1 student to staff ratio.
Essential Functions:
The Senior Mental Health Therapist is a licensed professional who will provide expertise in behavioral science and mental health related services, serving as an integral part of The Pines interdisciplinary team. The therapist will be the primary mental health provider for an assigned cohort of students and will be responsible for providing individual, group, and family therapy. The Pines will operate within a DBT framework, and the therapist will be comprehensively trained in DBT and will be directly involved with the implementation and utilization of DBT as a core component of treatment. The therapist will work closely with students, family members, and external parties to ensure comprehensive therapeutic care, and to support thorough transition and future planning. The therapist will also provide expertise by conducting behavioral assessments within their purview, consulting, counseling and supporting crises as needed, as well as designing and developing treatment plans and interventions that meet the individualized needs of students. The therapist will also work some evening and weekend hours on a rotating basis and be part of a rotating clinical on-call system. Having access to confidential information, discretion and independent judgement is required.
Conducts comprehensive assessments and reassessments of a student's behavioral and emotional needs within the approved parameters of their expertise
Provides crisis management and/or classroom push in/pull out support as necessary
Utilizes a variety of treatment approaches, including milieu approaches, that are safe, creative and individualized to a particular student's needs
Functions as a member of an interdisciplinary team to support the comprehensive treatment of students in The Pines
Engages students and families in treatment planning to set achievable and measurable goals and supports the identification and execution of interventions designed to support the attainment of those goals
