Provides training to family members, caregivers, and staff members. Plans and develops programs for restorative and wellness programs. Supervise physical therapist assistants. Actively functions and participates as a member of the interdisciplinary team. Provides visits and/or treatment in various settings, including day centers, Participants homes, skilled nursing facilities, or hospitals. Communicates Participant changes to team members as appropriate. Participates in the coordination of 24-hour care delivery. Must develop and maintain a strong working relationship with facility administration and staff, contract providers, other healthcare professionals, participants, caregivers, and families. Participate in Quality Initiative training as needed. Other duties as assigned. Required Skills/Abilities: Must be able to deal tactfully and patiently with other staff, participants, family/caregivers, visitors, government agencies/personnel, and the public. Must be able to adapt well to change. Need to have exceptional time management and problem-solving skills. Must possess the ability to make independent decisions when circumstances warrant such action. Must have the cognitive ability to process and perform tasks for Participant safety. Reliable transportation and a clean driving record. Ability to prioritize work and follow through on assigned tasks. CPR and First Aid Certification. Excellent written and verbal communication skills. Education and Experience: Must hold a current PT license within the state you are practicing in. One year of experience working with the frail and elderly population or willing to participate in extensive training as needed. Demonstrate clinical competency with the geriatric population. Requirements: Successful completion of background check, physical, drug screening, TB Screening. Physical Requirements: Sits, stands, bends, lifts, and moves intermittently during working hours. Is subject to lifting, carrying, and supporting participants and equipment. Must be able to drive a motor vehicle. Is subject to frequent interruptions.
