Duties:
Patient Evaluations (past medical history, exam, relevant imaging) Thoroughly and appropriately evaluates referred patients to assess their need for physical therapy services and the best possible physical therapy treatment plan.
In coordination with other appropriate clinical team members, develops realistic, best-practice rehabilitation program using appropriate therapeutic modalities designed to meet the patients' personal, vocational, recreational, and medical needs.
Orients patients to their individual physical therapy programs to assure their understanding of the treatments, mutually agreed upon goals, and approximate anticipated length of the treatment.
Establish Plan of Care
Administer prescribed modalities, therapies, and exercise programs ( Manual therapy, laser therapy, dry needling, pelvic floor, chronic pain mm)
Regularly reassess Patients to determine effectiveness/progress
Patient education as to importance of continuation of therapies, diagnosis, and treatment options
Coordination of treatment plan with patient and other team members
Completes and maintains accurate and timely documentation
Submits accurate charges for services and supplies
Collaborate and supervise physical therapy assistants (PTAs) to include direction of tasks to be completed

Position Summary Physical Therapist:
Plans and provides physical therapy services to clients in their homes in accordance with the Plan of Care and Agency policies and procedures. Required Experience Physical Therapist:

Responsibilities Physical Therapist:
Physical Therapist performs physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care.
Completes initial assessments within forty-eight (48) hours of referral unless other arrangements are made.
Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for speech therapy services unless other arrangements are made.
Develops a therapy care plan to provide direction to other health team members involved in the plan of care. Reviews and updates per Agency policy and client need.
Documents all findings, plans, interventions, and outcomes and submits documentation to the Agency within seven (7) days or per Agency policy.
Perform other duties as assigned.

This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. Provides thorough, timely, professional and consistent patient care documentation through the electronic medical record (EMR); evaluations, re-evaluations, progress notes, daily notes and D/C summaries per company policies; Documentation demonstrates minimal to no grammatical errors; Documentation clearly indicates medical necessity and clinical progression towards completion of care.
Contribute to success and growth of clinic through specialty programs and community outreach.
Complete all requirements to maintain up to date physical therapy license including continuing education.
Assist administrative staff with billing and reporting inquiries.

* Coordinates the daily operations of the department including but not limited to: * Managing calls to the department * Monitoring patient flow and patient satisfaction * Ensuring that department equipment is maintained and secures repairs as necessary
* Coordinating provider and patient schedules
* Demonstrates basic knowledge of physical therapy modalities.
* Responsible for the performance evaluation/career development reviews and competency evaluations of assigned staff.
* Establishes and monitors department specific protocols, adhering to GLBHC protocols including clinical, infection control, and safety protocols.
* Responsible for the completion of tasks as needed for the overall operation of the center including correspondence and missed appointment notices.
* Responsible for the tracking or referrals to the department and communication with referring providers.
* Participates in quality improvement measures including auditing and implementation of improvement strategies.
* Verifies insurance coverage, conducts income determinations, and assigns payment categories and updates patient information. Inputs data in computer in accordance with "Patient Accounting Manual" and GLBHC billing policy. Follows front desk manual for procedures.
* Maintains the clinical environment.
* Acts as a community and interdepartmental liaison to promote services offered.

As a Physical Therapist, you'll contribute to our success in the following ways:

* Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients' functional needs and adjusts care plans to ensure optimal patient outcomes.

* Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
