Position Summary
We are seeking a licensed Physical Therapist (PT) to join our hospital-based rehabilitation team. This position offers the opportunity to work with a diverse patient population across acute, skilled nursing, and outpatient settings. The PT will evaluate, develop, and implement individualized treatment plans to restore mobility, strength, balance, and functional independence making a meaningful impact on patients' recovery and quality of life.
Key Responsibilities
Perform comprehensive evaluations and develop individualized plans of care based on patient goals and clinical findings.
Provide evidence-based physical therapy interventions to address mobility, strength, balance, pain, and functional limitations.
Educate patients, families, and caregivers on home exercise programs, safety, and functional mobility techniques.
Collaborate closely with an interdisciplinary team, including OT, SLP, nursing, and case management, to ensure coordinated and patient-centered care.
Document evaluations, treatments, and progress notes accurately and timely in the EMR system.
Participate in discharge planning and provide recommendations for continued therapy, equipment, or home modifications.
Contribute to program development, implementation, and quality improvement initiatives within the rehab department.
Qualifications
Graduate of an accredited Physical Therapy program.
Current Georgia Physical Therapist license (or license eligibility).
Prior experience in hospital or outpatient settings preferred but not required (new grads welcome).
Strong clinical reasoning, documentation, and interpersonal skills.
Passion for providing exceptional care to patients in rural and community-based settings.

Responsibilities
As a Home Health Physical Therapist, You will:
Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
Maintain communication among the treatment team, physician, patient, and family members as applicable
Maintain thorough and timely documentation of services provided to ensure proper billing
Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act
Qualifications

1.
