The health care system also supports 10 community-based outpatient clinics (CBOC), located in Bismarck, Devils Lake, Dickinson, Grafton, Jamestown, Grand Forks, Minot, and Williston, N.D., as well as Fergus Falls and Bemidji, Minn. During the last two years, outpatient visits for the healthcare system and the CBOCs have steadily increased to well over 100,000 per year. The health care system had over 2,000 admissions into the hospital each year over the last two years. The CLC provides extended care through admissions of over 400 patients each year during the past two years. Responsibilities: Emergency physicians shall provide immediate medical evaluation, examination, diagnosis, treatment, and/or referral of patients who present for care in the ED "Present for care" includes patients referred to the Emergency Department by other medical providers in the VA Healthcare Facility system (such as outpatient clinics, outlying clinics, ED Fast track, inpatient, etc.). The ED physician shall provide professional advice to other VA providers with regard to professional management of patients being treated and/or hospitalized. The Contractor physician shall be a resource for contract providers and manage patients in the fast track portion of the Emergency Department as well. The Contractor physician care shall cover the range of emergency room care typically provided in an urban civilian practice of similar size. The following list is an example of the types of cases, but is not all-inclusive: Major trauma, including fractures of multiple extremities, stab wounds, open wounds of the chest and abdomen, head trauma, suspected spine injuries, lacerations, amputations; Major medical emergencies such as cardiac arrest, myocardial infarction, respiratory failure, shock; Obstetrical emergencies such as deliveries of single or multiple births, acute pregnancy complications, vaginal bleeding; Our Labor and Delivery Unit is on Long Term Divert. When possible, we have an OB provider (CNM, FP/OB) on resource call. Pediatric emergencies, including neonatal resuscitations; Acute psychiatric problems, including suicide attempts; Lower-acuity complaints including viral illnesses, other infections, minor orthopedic problems, minor trauma, alcohol abuse (i.e., "urgent care" patients);. Other miscellaneous medical problems both acute and chronic. Requirements: Unrestricted license to practice medicine in any U.S. State or Territory ABEM-Certification preferred, but other Board Certifications are acceptable with no less than 2 years of full-time ER experience in a high volume ED (Other qualifying certifications: Internal medicine, Family practice, Internal medicine/pediatrics, general surgery.) This is a full time position Monday - Friday with a combination of clinic and surgical assisting responsibilities (Generally: 60% Clinic / 40% Surgery Assisting). Call responsibilities are a part of this role (Typically: 1 in 9). ESSENTIAL DUTIES AND RESPONSIBILITIES Ensure comprehensive, professional, and friendly care to orthopaedic patient needs. Perform complete, comprehensive and problem focused history and physical exams. Order necessary radiologic studies and laboratory tests. Interpret and correlate subjective data as well as laboratory and diagnostic procedures to formulate a differential diagnosis and establish a working treatment plan for Orthopaedic problems within scope of practice. Implement therapeutic intervention for specific conditions when appropriate. Exercise judgment on problems requiring consultation, referral, or evaluation by the collaborating Orthopaedic surgeon or other health care professionals. Assist the Orthopaedic surgeons during surgical procedures. Provide direct patient treatment, including but not limited to: applying and/or removing casts/splints, simple wound care and repair, injections of joints, tendons, trigger points and bursa, aspirations of joints, application of braces, splints, and other DME. Provide patient and family instruction on surgical after care. Complete patient's medical record documentation in a thorough and timely manner Act as role model through support of The Bone & Joint Center's mission Responds to physician and nursing concerns timely Maintains patient records confidentially. Collaborates with the entire Bone & Joint Center team and provides professional recommendations regarding patient care. Maintains a safe and clean working environment by complying with procedures, rules, and regulations, and adhering to infection-control policies and protocols. Maintains professional appearance and personal conduct. Proficient in prioritizing responsibilities according to patient and physician need. Skilled in educating patients and coordinating patient care. This is an open continuous ad and candidates will be referred until filled. The PA practices general medicine with a focus on community-based outpatient clinical care. Approximately 90% of the PA's time is dedicated to developing and maintaining an interdisciplinary approach to the direct care of general medicine clinic patients. ii. The PA independently follows his own panel of general medicine patients, functioning as their primary care provider. This primary care/gatekeeper's role is incumbent with a high degree of responsibility and independence. iii. The location of the CBOC away from the Medical Center means that the PA's assessment skills and breadth of knowledge must be superior. In addition, the employee must be a highly effective communicator to negotiate with consultants at the Fargo VA HCS regarding the triaging of patients he refers to. iv. Provider will be required to be a designated women's provider and complete the mandatory VHA training. v. Providers may be required to travel to the CBOC's or provide gap coverage as determined by the supervisor. vi. All providers will meet the work relative value unit (RVU's) as determined by the national office for primary care providers. vii. Typical work hours are Monday to Friday 8-4.30 PM. Alternative work hours, if desired, should be agreed upon at the time of joining. Each full-time provider is required to work a minimum of 80 hrs. per pay period (every two weeks) unless on leave status. viii. Providers will need to act as surrogates for the patients of their colleagues, when one or more of their colleagues are on leave. ix. Alternative or collateral duties may include management of a subsection of primary care patients, the subsection may be defined either by a disease status (e.g. spinal cord injury patients), or demographic status (women veterans, OIF/OEF veterans, employees etc.). Typically, these duties are designated at the time of joining but occasionally during employment, participation in special projects pertinent to assigned section may be necessary as the projects become available through strategic or other initiatives. Primary providers in the outpatient setting are typically assigned a panel of patients to manage. Panel size currently is determined by utilizing VHA PCMM software. Panel size is determined at the National level, through a complex formula which extracts local information regarding the acuity of assigned patients, available support staff, and the number of exam rooms. Each provider's panel size is specific to their practice and setting. xi. All other duties as assigned. xii. Clinical reminders should be addressed as medically appropriate.
