Responsibilities: · Screen and assess each patient for co-existing medical problems to assess risk factors affecting appropriate placement for treatment. · Administer medications/treatments as ordered in accordance with state and federal regulations. · Ensure that adequate recording of the medication administration transaction has occurred. · Ensure that the treatment plan of the physician and/or medical director is being followed. · Facilitate and educate patients in a group process to include chemical dependency issues, infectious disease, medical and mental health issues. · Administer, interpret, and keep accurate documentation of Mantoux test for teammates. · Assess patient response to treatments and medications, interactions, and effectiveness. · Document and summarize individual nursing care for each patient and communicate changes, progress, and/or immediate medical care needs to the provider for follow-up consultation. · Orders medical supplies, maintains a proper inventory and record of use as per Regulations · Maintains all medical records including individual charts, daily medication charts, dosage records, progress notes, doctor's orders, etc., as required. · Makes appointments for patients to see physician; obtains lab workups and checks with patients to see that appointments are kept. · Counsel's patients regarding personal hygiene, abnormal lab results, HIV/AIDS testing, and all medical matters including changes in treatment. · Discusses with physician and team important changes in treatment or noted patterns of behavior affecting treatment. · Provides referral for specialized care at local health centers (e.g., Planned Parenthood, prenatal clinic, HIV/AIDS testing sites, etc.) and follows-up on patients' attendance and treatment. · Assists in organizing transfers of patients to and from different levels of care. · Assist in training teammates in medical conditions, responses, and patient care. · Maintain continuing education credits as required by the Board of Nursing · Attend team meetings and complete all training courses timely as required. · May assists doctor during physical examinations. · Keeps medication room clean and orderly; maintains medical equipment. · Attend team meetings and complete all training courses timely as required. · Other duties as assigned.
