Your Qualifications:
Board Certified by the ANCC as a PMHNP

* Licensed or eligible for licensure as an Advanced Practice Nurse Prescriber in the State of Wisconsin. Experience in provision of an eclectic problem-solving approach to clients seeking pharmacological support and problem resolution. Familiarity with a range of issues such as poverty, community, family violence, HIV, human sexuality, AODA, criminal justice systems, chronic or terminal illness and pain, and parenting skills, are desirable.

Essential Job Duties and Responsibilities:
Leadership
The NP will demonstrate nursing leadership and positively influence the profession of nursing.
The NP will contribute to a culture of excellence, collaboration, integrity, compassion, and accountability.
The NP will role model excellence in clinical professional practice and work alongside staff to reinforce learning and provide specialist assistance when required.
The NP will collaborate with other APNPs in the region and participate in peer support and mentorship.
The NP will communicate effectively, timely, and appropriately at all times.
Advanced Practice
The NP will be responsible for the medical aspect of the patient care programs and activities with members of the local medical community in collaboration with the Medical Director and member of the medical team.
The NP will review and collaborate with the Medical Director regarding appropriateness of patients at time of hospice admission.
The NP will round on designated patients at the Jack and Engrid Meng Hospice Residence when directed.
Education, Development, Consultant
The NP will be available to consult with team members on an as needed basis for clinical issues relating to the patient/family plan of care.
The NP will serve as a consultant with the patient's attending provider as requested.
The NP will provide clinical expertise, education, and training on pain and symptom management and other palliative related topics to Unity staff.
The NP will serve as a hospice and palliative resource with attending and consulting providers as necessary regarding pain and symptom management, chronic disease management, patient prognosis, and palliative treatments in collaboration with the Medical Director.
Quality and Compliance
The NP will provide face-to-face recertification visits and will assure the accurate and timely documentation of those visits to assure seamless care of Unity's hospice patients.
The NP will identify practices of concern, risk, and learning opportunities.
The NP will assist in the development and delivery of comprehensive, quality plans of care for patients/families in accordance with Unity program standards, collaborating with other members of the IDT.
The NP will accept the role of overseeing the patient's end-of-life care needs if/when needed or requested.
The NP will ensure the accuracy and timeliness of documentation to support CPT codes and billing.
The NP will adhere to Unity's policies regarding the protection of all health information.
The NP will perform other duties as assigned by the Medical Director or designee.

Requirements
1. A master's degree in nursing or equivalent (with prescribing authority)
2. Minimum of 3 years of professional advanced practice nursing experience.
3. Advanced Practice Clinician shall maintain an unrestricted certification as a nursepractitioner in the State of Wisconsin.

Position Summary:

Essential Job Functions:
* Obtain health history and perform physical examinations.

* Perform minor surgical procedures.
* Communicate referrals for patients requiring services not offered at N.E.W. Community Clinic taking into consideration available community resources and the financial challenges associated with our patients.
* Review incoming diagnostic reports (e.g. lab, x-ray, EKG) in electronic medical record and follow-up in a timely manner.
* Comply with the N.E.W. Community Clinic Infection Control plan while ensuring compliance by support staff.
* Document patient care, services, and referrals using electronic medical record system.
* Possess a thorough knowledge of the requirements for reporting communicable diseases in the State of Wisconsin and make appropriate referrals.
* Demonstrate adaptability in responding to changing trends in nursing and share knowledge and experience.
* Communicate effectively with patients, staff, outside agencies, students, volunteers, and other visitors to the clinic.
* Protect confidentiality as directed by HIPPA regulations.
* Maintain regular clinical review meetings with collaborating physician.
* Work effectively as a member of the health care team.
* Carry out professional nursing role as described in the WisconsinNurse Practice Act and the American Nurses Association Social Policy Statement.
* Maintain professional continuing education certification requirements and unencumbered professional license.

* Participate and work with clinic leadership in accepting students from various school programs to shadow in the clinic as needed.
