Ensures sustained high quality of care and services to persons with mental health, substance use and/or developmental disabilities. Complies with company, state and federal policy. Required Education/Experience Education- For all populations served, Care Managers must meet at least ONE of the following qualifications to meet the criteria as a Qualified Professional (QP), per 10A NCAC 27 .0104 A MH/SU license (including associate-level), or are certified by the NC Substance Abuse Board An RN AND have four years of full-time experience working with the MH/SU/IDD population* A master's degree in a human service field** AND at least one year of full-time experience working with the MH/SU/IDD population* A bachelor's degree in a human service field** AND at least two years of full-time experience working with the MH/SU/IDD population* A bachelor's degree in a non-human service field AND at least four years of full-time experience working with the MH/SU/IDD population* Experience- Care managers need to have care coordination, care management, or case management experience. Best Practices - Demonstrates a passion for positive change by continuously improving and defining innovative care management interventions. Stays up to date with current best practices and keeps the team continuously informed about best practice innovations. Cross-functional Collaboration - Collaborates cross-functionally with other Integrated Care Team members, community organizations and RHA departments to ensure success of Care Management implementation. Person-Centered Approach - Maintains a person-centered philosophy focusing on an individual's strengths, needs, and preferences that promote health, wellness, and safety. Fosters a person-centered culture with Care Management Teams to optimize individual education around self-management, disease management, and integrated healthcare planning. Quality Assurance and Use Of Data - Implements the strategic use of data to support delivery of effective, integrated, and well-coordinated care management services.. Ensures individual data sharing and storage consistent with state and company policies. Maintains a Safe and Healthy Environment- Implements all organization health and safety policies and procedures. Financial Performance - responsible for documenting services provided in a timely manner in order to be appropriately accounted for reimbursement purposes Ensures proactive and effective communication - Establishes and maintains frequent and regular communication with persons supported, family members, other team members and colleagues. Directs the confidential and immediate investigation into allegations of abuse, neglect, exploitation, theft, fraud, waste, misconduct, and violations of ethics and compliance practices according to company policy. Ensures protections of individuals served during investigations. Ensures all external reporting requirements are followed. Ensures adequate follow- through on recommendations from investigation reports. Participates as assigned as designated members to Quality Improvement Committee, Quality Assurance Committee, Quality Management Team, Safety, Task Force and Human Rights Committee meetings. Performs all other related duties as assigned. The Painter is an in-house position within the Facilities & Fleet (F&F) Department responsible for addressing painting and surface-finishing needs across RHA Health Services facilities. This role supports the upkeep, safety, and appearance of residential homes, facilities, and administrative spaces by performing high-quality interior and exterior painting, surface preparation, and minor related repairs. The Painter works collaboratively with Facilities leadership to assess project needs, recommend materials, and execute work efficiently while minimizing disruption to operations and individuals served. This position requires travels to RHA locations. KEY RESPONSIBILITIES: Perform interior and exterior painting projects across RHA residential homes, facilities, and administrative buildings. Prepare surfaces for painting, including cleaning, sanding, scraping, patching, and priming walls, ceilings, doors, trim, and other surfaces. Conduct minor drywall repairs, including patching holes, cracks, and surface imperfections prior to painting. Perform light carpentry or minor maintenance tasks directly related to painting projects, as appropriate. Assess job sites and develop material estimates and scope of work, including recommended paint types, quantities, supplies, and preparation needs for each project. Assist with assembling supply lists and coordinating paint and material purchases in alignment with departmental processes. Safely operate and maintain painting tools and equipment, including ladders, sprayers, rollers, and brushes. Ensure all work meets quality standards, safety requirements, and infection control expectations appropriate for healthcare and residential environments. Maintain clean and organized work areas during and after project completion. Project scope may vary from full facility paint refreshes to targeted patchwork and touch-up projects. Performs a variety of administrative, training and personnel functions. Supervises and coordinates activities of Direct Support employees and manages operations, staffing and budget of assigned facilities. Conducts unit investigations for compliance and allegations of abuse, neglect, and exploitation. Coordinates and monitors unit's quality assurance system. Assists Administrator with the development of services and oversight of the unit. Takes rotating on-call responsibility. Relies on experience, knowledge, and judgment with limited direction and supervision. DUTIES AND RESPONSIBILITIES: Develops and maintains a system for assuring integration of basic developmental care and programming services. Supervises and coordinates activities of Direct Support employees and manages operations, staffing and budget of assigned facilities. Interviews and hires Direct Support Professionals Assists with creating employee schedules ensuring that there is always adequate staffing coverage. Works closely with clinical and professional healthcare staff to maintain communication and provide feedback, standardizing procedures, and expediting the implementation of person-centered plans. Provides shift coverage in times of acute shortages of direct support staff. Provides on call coverage for people supported they support and services as needed. Provides after-hours and on-call coverage as part of the direct care rotation Works closely with Qualified Professional and other clinical/professional staff to maintain communication and provide feedback, standardize procedures, expedite Person Centered Plan implementation and workflow and to improve efficiency of team Partners with the Worker's Compensation team to ensure prompt reporting and investigation of employee injuries, ensures employee receive needed safety training and coaches employees not working safely and properly Periodically monitors all shifts and basic developmental care to ensure that legislative standards are met. Constantly monitors activities to ensure that health and sanitary standards are met; corrects any deficiencies noted by internal/external authorities. Assists in the Interdisciplinary Team Process, as needed. Participates in the Quality Assurance Assessment system which may include; Mealtime Assessment, Environmental Assessment; Interaction Assessment and Third Shift Assessment. Interviews, hires, coaches and terminates employees as needed. All other duties as assigned. SUPERVISORY RESPONSIBILITIES: This position supervises Group Home Supervisors and Direct Support Professionals MINIMUM QUALIFICATIONS: A high school diploma or equivalent One year of experience working with people with developmental disabilities. Excellent verbal and written communication skills. Flexible schedule and availability to provide on-call coverage as needed. CPR, first aid, NCI and/or MedTech certification, a plus Must be at least 18 years of age PHYSICAL DEMANDS AND WORK ENVIRONMENT: Regularly required to lift 20 lbs. Must be able to lift a minimum of 50 lbs. Must be able to pull a minimum of 20 lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing, and lifting. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to talk and hear.
