May conduct a psychiatric social work assessment according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Develops treatment plans with appropriate and competent interventions. 2. Responsible for psychosocial assessment and intervention in crisis situations that often involve issues of suicide, homicide, family violence, elder or child abuse. 3. Demonstrates sound clinical judgment in assessing the patient's needs for long-term care, including evaluating the home and family situation, helping the patient and family to develop an in-home care plan, exploring alternatives to in-home care and arranging for placement if necessary. 4. Provides treatment, therapy, psycho-education and/or counseling utilizing individual, family or group modalities. 5. Provides information, referral and creative resource acquisition for specific individual and family needs both within the Hospital and in the community as indicated. Assists patients/families to understand psychosocial factors impeding their maximal utilization of hospital and community resources. 6. Collaborates with and provides psychosocial consultation to the health care team to promote a smooth, coordinated plan of care. Aids the team in understanding and integrating the significance of psychosocial factors in relation to patient's illness, treatment and recovery. Identifies psychosocial issues that may impede progress. Makes recommendations to the team regarding patient/family care and management. Assists health care team to assist patient/family level of understanding to make informed decisions. 7. Provide medical social services to the patient's family member or caregiver on a short-term basis when necessary to remove a clear and direct impediment to the effective treatment of the patient's medical condition or rate of recovery. 8. Treats patients and families with dignity and respect at all times. Offers time for questions and answers; demonstrates empathy in communication while working toward optimal health goals. 9. Provides outreach, case finding/screening for high-risk issues that may impact on patient's progress, participation in plan, discharge or ability to utilize resources. 10. Assists with discharge planning issues to ensure continuity of care. Formulates, coordinates and implements the psychosocial components of outpatient care. 11. Consults, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources. 12. Serves as a patient/family advocate with a variety of systems both internally and externally as required. May be called upon to testify in court. 13. Ensures clinical documentation is complete and according to departmental standards. Performs other administrative documentation and record keeping, such as accountability and statistical reports, billing forms, student intern evaluations, performance evaluations, etc. in complete and timely manner. 14. Initiates policy and program development in specific service area or team, utilizing knowledge of state-of-the-art programs and promoting social work values such as patient self-determination and social justice. Initiates, participates in and supports policy and program development in the Department of Social Work Services. 15. Actively engages in the regularly scheduled meetings, programs and activities of the Department. Serves on departmental committees. Represents the profession and Department in hospital and community meetings and committees. 16. Participates in social work professional education, supervision and teaching activities; supervises or trains staff social workers, student interns, house staff, nurses, medical students, and volunteers as required. 17. Maintains collaborative, team relationships with peers and colleagues in order to effectively contribute to the working groups achievement of goals, and to help foster a positive work environment 18. Continually monitors, assesses and modifies his/her own social work practice and programs. Conducts quality improvement activities and research regarding service delivery and effectiveness of interventions, as required. 19. Conducts or participates in professional organizations, grant writing, teaching, public presentations, seminars and workshops. May supervise staff social workers, social work associates, social work interns, parent consultants, house staff, nurses, medical students and volunteers as required. 20. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures. Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines. Physical Requirements: 1. Normal office setting. 2. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process. 2. Knowledge of casework process and community resources in relation to health (including mental health), illness, and disability. 3. Knowledge of current social work theory and practice, the ability to apply treatment and case management methods, and familiarity with community issues and resources. 4. Understanding of organizational theory and practice in large systems and the knowledge of social and health care policies and systems. 5. High degree of diagnostic, treatment, interpersonal, organizational, and communication skills. 6. Analytical skills required assessing patient needs, to develop associated treatment modalities and to provide effective counseling. 7. Computer skills and knowledge of general office programs, spreadsheet programs, database programs, and presentation programs (Word, Excel, Access, PowerPoint). 8. Ability to read/write and communicate in English. 9. Excellent interpersonal skills. 10. Ability to function well in very busy situations. 11. Responsible and reliable. 12. Good organization skills. 13. Ability to engage a wide variety of patients and collaterals in the acute treatment and discharge planning process. 14. The 6 East and 6 West Child Unit is a 21 bed secured psychiatric unit providing care for children between the ages of 3 and 11. The unit is located at our CHA Somerville Campus. A multi-disciplinary approach is used to provide patient and family centered care. A continued emphasis on restraint reduction is ongoing and treatment focus is on assessment, stabilization, and after care treatment planning. Our approach is trauma informed, patient and family centered, and evidence based. Job Purpose The Milieu Counselor is a member of the multidisciplinary team who, under the supervision of a registered nurse, is responsible for a safe and structured service as well as attending to the individual needs of patients in crisis in an inpatient psychiatric setting. The role of a Milieu Counselor will assist with ADL'S lead or co-lead groups, perform safety checks, 1:1 meetings with patients, and will accurately chart patients B/P, TPR & weight. Notifies RN of any changes in vital signs. Accurately measures and documents patient's intake and output. Assists patients following through with ADL's (Activities of Daily Living), maintaining nutritional and hydration needs, and assisting and monitoring the patient's needs and other tasks as assigned by the charge RN. Qualifications/Experience: High school diploma required; B.A, B.S., or M.A. preferred or equivalent years of human service experience. Position Responsibilities: * Provide specialized case management and supportive services coordination for all CPEAN clients, in collaboration with referring agencies or other community partners as appropriate * Manage CPEAN's elder abuse prevention phone line and email address, and timely follow up on any referrals or inquiries * Perform intake, initial and ongoing assessment, and discharge planning utilizing specific person-centered, trauma-informed tools * Create Supportive Services Plans (SSPs) for all CPEAN clients, and track client progress and outcomes using aforementioned tools; monitor types and frequency of services utilized * Work collaboratively with Case Manager and Housing Placement Specialist to provide supportive services directly to CPEAN clients as needed and dependent on qualifications and bandwidth; examples include - accompanying clients to appointments, assisting clients with completing benefit applications (paper and electronic), providing trauma-informed counseling, etc. * Conduct outreach to referral sources (including but not limited to protective service agencies, senior centers, hospitals, long-term care facilities, rehab facilities, domestic violence shelters, homeless shelters, faith communities, etc.) to foster collaborative relationships, capture appropriate referrals, continuously improve referral/screening/ admission procedures, etc. * Work collaboratively with Case Manager and Housing Placement Specialist to assess and address client housing needs * Assist Program Director as needed with facilitating multidisciplinary teams and elder justice task force meetings in and around Boston, as well one-on-one meetings between CPEAN and select Adult Protective Services (APS) agencies * Create and deliver (solo and/or with CPEAN colleagues and/or community partners) educational presentations and trainings on elder abuse prevention and related topics * Assist Program Director in creating, reviewing and enhancing CPEAN standardized documentation, policies and protocols around intake, care/service coordination, discharge, and post-discharge follow-up * Participate in CPEAN donor events and funding solicitations as needed * Represent CPEAN in committee and coalition meetings as appropriate, report on challenges and explore resources * Perform other related duties as assigned or as responsibilities dictate. Qualifications: * Minimum 3-5 years of experience, including both direct care and case management and/or care coordination; experience working with survivors of trauma strongly * Must have a Masters degree in social work. Must be currently licensed to practice in the Commonwealth of Massachusetts. Collaborates in the diagnosis and treatment of patients; shares information and participates in the formulation of treatment plans. Coordinates and implements various aspects of milieu environment to provide a therapeutic experience for patients. Collaborates with other staff members to determine whether a patient represents a risk to himself/herself and/or others. Works with other unit personnel with crisis intervention techniques. Provides supportive care to patients, such as participating in conversation, recreational activities, and basic living activities. This may include assisting patients with bathing, eating, and other daily living activities. Sets limits, confronts counterproductive activities and behavior, and reassures and orients patients. Encourages and facilitates appropriate interactions between patients. Records all appropriate information on patient charts; prepares regular reports describing each patient's mental status and functioning, as well as significant interactions. Reports and documents overt, covert, verbal, and non-verbal communications by patients. Provides support and education to family members and significant others regarding the patient's status. Works with representatives of outside agencies in obtaining or disseminating information on individual patients. Participates in discharge planning and referral, if necessary. EDUCATION MINIMUM EDUCATION Bachelor's degree in psychology, Social Work, or related field. EXPERIENCE MINIMUM EXPERIENCE 1-2 years of experience as a Mental Health Tech CERTIFICATIONS/LICENSURE PREFERRED CERTIFICATION/LICENSURE BLS from American Heart Association Trusted Resource Associates (TRA) Job ID #500156.
