Overview

General Position Description:
Medical Social Services will be provided by a qualified socialworker under the direction of a physician. Medical Social work services will be based on the patient's psychosocial assessment and the patient and family's needs. The Master of Social Work is responsible for supporting and supervising Medical SocialWorkers to deliver high-quality care consistent with standards of practice. Be readily available when social services are being provided.
The supervising Master of Social Work must hold documented conferences with the Medical SocialWorker regarding patient care.
The Master of Social Work is responsible for determining the frequency of the conferences consistent with accepted standards of practice.
Assesses and completes the Comprehensive Assessment within five (5) days of the patient's election of hospice care to identify patient and family's psychosocial, financial, environmental and community needs as evidence by POC, documentation, clinical records, IDG/T meetings and community resource referrals.
Assists with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family's needs.
Meets mandatory continuing education requirements of the Hospice and licensing board.
Demonstrates commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
Provides patient and family/caregiver counseling around issues of end-of-life, death, and grief.
Attends IDG/Team meetings, participate in the patient care planning process and coordinating and collaborating with the IDG to promote coordination of patient care.
Provides an environment that promotes respect for patients and their privacy and property.
Provides services utilizing infection control measures (OSHA)
Timely submission of all required paperwork.

The Family Centered Treatment Therapist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Qualifications
Minimum of a Master's Degree in human services or related field is required.

What You'll Do
Clinical Supervision & Leadership
- Provide high-quality clinical supervision to school-based mental health counselors (individuals pursuing or maintaining licensure)- Ensure ethical, evidence-based clinical practice aligned with Tennessee licensure standards and best practices- Support clinicians with case consultation, risk assessment, crisis response guidance, and treatment planning- Model strong clinical judgment and professional boundaries in school settings
Medicaid Billing Oversight & Compliance- Oversee Medicaid-aligned service delivery and documentation for school-based mental health services- Ensure clinicians understand and adhere to: - Insurance-eligible services (including Medicaid) and medical necessity standards - Proper session structure, progress notes, and treatment plans - Timely and accurate documentation requirements- Sign off on all Insurance claims for individuals under your purview who are not yet fully licensed.- Review clinical documentation to support clean claims, audit readiness, and compliance- Partner with Thrive's billing and administrative teams to: - Address denials, corrections, and documentation gaps - Support continuous improvement in reimbursement processes - Align clinical practice with payer requirements without compromising care quality- Provide training and ongoing guidance to clinicians on Medicaid/Private Insurance billing expectations in school-based settings
Quality, Compliance & Documentation- Oversee clinical documentation to ensure accuracy, timeliness, and compliance with: - Tennessee licensing requirements Medicaid/Private Insurance and payer standards (where applicable) - School-based service expectations - Support audit readiness and continuous improvement efforts
School & Community Partnership- Collaborate with school administrators, counselors, and support staff to ensure seamless integration of mental health services- Provide consultation and guidance on school-based mental health best practices- Support positive, solution-oriented relationships with families and caregivers
Team Development & Culture- Mentor clinicians through reflective supervision, feedback, and professional development - Contribute to Thrive's culture of accountability, growth, and mission alignment- Support onboarding and training of new mental health staff- Partner with Thrive leadership on program development and service expansion in the Chattanooga region
What We're Looking For
Required Qualifications
- Active, unrestricted Tennessee licensure (LPC-MHSP, LCSW, LMFT, or Psychologist)- Meets Tennessee requirements to provide clinical supervision- Minimum 5+ years of clinical experience, with at least 2 years in a supervisory role- Experience working with children and adolescents- Familiarity with Medicaid/Private Insurance billing and documentation requirements
Preferred Experience- School-based or community mental health experience- Experience supervising early-career clinicians or provisionally licensed providers- Comfort operating in fast-paced, collaborative environments

Responsibilities
As a Home Health SocialWorker, You will:
Guide and counsel individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
Assist clients in obtaining government funds, education, or treatment
Secure information, such as medical, psychological, and social factors contributing to the client's situation, and evaluate these and the client's capacities. Ask clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments, to determine the nature and degree of the problem
Help patients and families through individual or group conferences regarding plans for meeting needs and aids clients in mobilizing inner capacities and environmental resources to improve social functioning
Help the client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and the client's part in creating them
Develop a service plan designed to restore the patient to optimum social and health adjustment within the patient's capacity
Qualifications
Master's Degree in Social Work
Five plus years of clinical social work experience
Valid and current Licensed Clinical SocialWorker or Licensed Medical SocialWorker license
Ability to make psycho-social assessments and develop and implement viable care plans
