The Department of Community Resources (DCR) through its Community Resource Center, supports the core departments of Mecklenburg County Health and Human Services (HHS) in doing their business through an integrated service delivery model of subject-matter experts from each core department and community partners who provide services to County residents in our HHS Community Resource Center. In addition, DCR is responsible for administering Child Support Services, Economic Services, MeckSuccess, HOMES, and Unified Workforce Development programs, as well as Integrated Health and Human Services Mail Services.
Position Specific Summary
The Child Support Case Coordinator position will support the Department Monday - Friday. The selected candidate will undergo a training period for the first couple of months upon acceptance of this role and successfully pass knowledge checks administered during that time.

Position Summary
Responsible for providing professional-level case management or case coordination services for child support services purposes. This is an entry level position and will manage child support cases in accordance with federal, state, and local laws and procedures.
Essential Functions
* Initiate, prepare, organize, and enforce child support orders in accordance with program requirements
* Monitor court-ordered payments to determine what action to take when the absent parent is in out of compliance with court order
* Mediate out of court, schedules for court and monitors cases for compliance
* Establish voluntary and judicial paternity and support
* Establish, modify and enforce child support orders, prepare cases for legal action and support orders by analyzing, monitoring, and reconciling accounts
* Prepare cases for legal action, including providing direct support to agency attorney in court; testifies in court and documents court disposition into the automated system

* May be required to staff shelters in the event of a disaster

Experience:

Mecklenburg County is the Lead Agency for Coordinated Entry (CE) for the Charlotte-Mecklenburg Continuum of Care. Coordinated Entry is the intake for the local homeless response system. It is part of the process to access homeless resources and shelter in Charlotte-Mecklenburg.
This SocialWorker I position conducts Coordinated Entry (both in person and telephonically) with individuals and families experiencing homelessness, or those at imminent risk of homelessness, to efficiently connect them to the best available (scarce) homeless shelter, temporary housing, or homeless prevention options in the community. It may require the provision of short-term Systems Navigation support to clients until which time they are connected to a homeless services case manager and/or their situation resolves.
This position also engages clients within a strengths-based, solution focused framework to identify their service needs and goals, and to facilitate referrals to community resources so that clients have a plan of action toward enhanced self-determination.
CE clientele is diverse and brings with them a wide range of experiences and concerns; and may be in different stages of mental health, substance use, and trauma recovery.
In addition to clients, the SocialWorker I requires the ability to effectively engage with community partners to provide on-site CE to clients wherever they may be (in places not meant for human habitation, hospitals, treatment centers, day centers, shelters, etc.)
Position Summary
Provides community-based services to clients, including evaluation, case management, counseling, and referrals. This position, under direct supervision, is responsible for determining the individual's specific problems and assisting in developing an action plan and/or mutual responsibility agreement.

The Case Coordinator provides case coordination activities such as obtaining updates from other providers, locating kinship placements, facilitating discharge planning, making referrals and linkages for needed services and supports, and participating in the child and family team process. This position serves youth in home, community, and office settings.
Principal Duties and Responsibilities
Provide care coordination activities such as researching community supports for the member and family and obtaining progress updates from other providers and supports.
Develop and maintain frequent contact with external and internal referral sources to assure smooth and timely transition of youth from Child ACTT services.
Function as a liaison internally and externally related to client care from admission to discharge.
Participate in and/or facilitate Child and Family Team Meetings.
Search for appropriate kinship placements for clients as needed. Work with stakeholders (DSS, parents/guardians, court counselors, care coordinators) to identify possible kinship options.
Works with internal stakeholders to determine appropriate discharge plans for youth in Child ACTT services and assist in making referrals and coordinating all follow up care appointments and services prior to discharge.
Facilitate a focus on readiness for discharge amongst staff and as evidenced by treatment plans and discharge plans that are an appropriate match with readiness.
Provide cross training in daily, weekly, monthly operations, Teaching Family Model, CPI and other elements as assigned.
Assist in the completion of outcome surveys, exit assessments, Wellness TOPs, and all discharge documents by day of discharge.
Assist with the maintenance of all required medical record documentation, securing current consents and permissions in advance of the expiration of existing documents.
Ensure proper completion of authorization documents needed for LOC change when clients transition internally or externally.
Provide on call coverage as part of an on-call rotation team as assigned.

Responsibilities:
This position is responsible for case management of two programs within Adult Services. The programs provide services to disabled adults age 18 and older, and are for Medicaid eligible recipients. The second program is the Special Assistance In Home program which provides essential support to low-income individuals who are at risk of entering residential facilities, and a cash supplement and additional support services enable them to remain in the home. Case management for both programs require comprehensive assessments which evaluates psycho-social, economic, environmental, physical and mental health factors. The socialworker develops a plan of care, mobilizes resources and monitors the plan of care for the patient in their own home. There is collaboration with multiple community professionals including medical personnel and community-based providers. The socialworker advocates for the client and also serves as the liaison with Adult Medicaid staff. The socialworker provides service intake several times monthly which includes submitting CAP referrals and the ability to educate the community on adult service programs and community resources. This position also requires participation in the energy programs with processing applications to determine eligibility. This position requires in home visits as well as seeing clients in the office. Other duties may be assigned as agency needs require.
