EXPERIENCE AND EDUCATIONRequiredEducationMaster's Degree in social work Experience1 year of experience in a health care setting preferable with homebound patients. Licenses and CertificationsCurrent license is required by the Texas State Board of Social Work Examiners. (DRIVERS) DRIVERS LICENSE TX Driver's License and proof of driver's insurance JOB DUTIESEnsures the delivery of quality social service for patients by identifying patient needs and developing social service programs to meet those needs. Participates in development and update of Plan of Treatment. Evaluates patient and home environment to determine social and psychological factors related to patient's health problems. Prepares and submits to agency psychosocial histories, clinical and progress notes. Observes and reports on patient/family reaction to treatment. Counsels' patient and/or family in understanding, accepting, and adjusting to patient's health problems and emotional care. Identifies, informs, and refers patient/family to available community resources. Coordinates discharge planning. Interprets to the community the role of social work in the Agency's programs. Participates in planning, development, and delivery of staff in-service education programs. Participates in Continuing Quality Improvement Program. Monitors adherence to policies, procedures, and protocols. Performs other related duties as assigned or requested.
