Job Essentials
Organize and prioritize daily work by reviewing new, current, and discharging patient needs in area(s) of responsibility. Complete documentation as required.
Gather psycho-social information for patients and caregivers.
Educate, communicate, and coordinate with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely intake and discharge, and to identify and resolve delays and issues.
Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care team, outside agencies and other resources as necessary.

Minimum Qualifications
Bachelors degree from an accredited institution (degree will be verified).
Current state licensure as a Social Services Worker.

State certification as a Clinician (e.g., Licensed Clinical Professional Counselor, Licensed Marriage and Family Therapist, Licensed Professional Counselor, or similar) in the state of Utah.
Master's degree or higher in psychology, counseling, social work, or a related field.
Extensive experience providing clinical supervision to mental health professionals, preferably in a community-based setting.
In-depth knowledge of evidence-based practices, treatment modalities, and therapeutic interventions for a range of mental health concerns.
Familiarity with Utah state regulations and requirements for mental health service delivery.
Strong leadership skills and the ability to guide, mentor, and support a team of support service workers.
Excellent communication skills, both written and verbal, with the ability to provide constructive feedback and guidance.
Exceptional organizational skills and the ability to manage multiple responsibilities simultaneously.
Proficient computer skills for documentation and record-keeping purposes.
Valid driver's license and reliable transportation.

Position Overview
As an LCSW with Mindful Mobile Medicine, you will:
Provide in-person psychotherapy services to older adults and other individuals living in private homes, assisted living, and independent living environments.
Collaborate closely with our Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver integrated, patient-centered care.
Assess mental health needs, develop treatment plans, and provide individual therapy.
Maintain thorough and timely clinical documentation.
Qualifications
Required: Active LCSW license in Utah
Required: Ability and willingness to work in the community (travel to homes and facilities)
Strong clinical skills and a compassionate, client-centered approach
