Essential Duties and Responsibilities:
Provides necessary social work intervention
Counsels individuals and families in crisis and help with issues such as poverty, loss and illness
Assesses patient's psychosocial needs, plans for, evaluates and modifies interventions to meet the established patient/client goals
Collaborates with all disciplines to plan and evaluate team goals for each patient/client
Facilitates patient/client and family referrals to community agencies when appropriate

May be required to respond to crisis situations, including domestic violence and child abuse
May be required to provide witness services and evidence in court cases
Visits schools to assess academic concerns
Works with government agencies to help clients apply for Medicare and Medicaid when appropriate
Arranges home visits to evaluate a patient/client health, home environment and family interaction/support
Assists patient/family/caregivers in planning for present and future psychosocial needs
Observes and reports patient progress and/or changes in patient status to physician and appropriate multi- disciplinary team members

Minimum Requirements:
Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education required
Active license as an LCSW in state of practice required
Minimum of one year of social work experience preferred
Complies with all relevant professional standards of practice

Current CPR if applicable

Program and Role Summary
Wayfinder's Sonoma Psycho Social Assessment program assesses resource families, including adoptive families, by completing resource family evaluations within county timeframes.
The primary focus on the SocialWorker, is to conduct resource family assessments for relative and non-relative families; and write the assessment in the county contract's allotted time. Psychosocial Assessment (PSA) SocialWorkers may also assist with finalizing the adoption for these families.
This is a part-time position budgeted for 29 hours/week with a varied schedule (weekend and evening availability, as needed). The PSA SocialWorker will provide support to both Contra Costa and Sonoma Counties, as needed.
Primary Responsibilities:
Assesses county-referred applicants to become an approved Resource Family.
Conducts a home environment assessment, and psychosocial assessment, and prepare a written report. Incorporate an assessment of an applicant's mental health and substance abuse into the risk assessment.
Provides ongoing assessments of family needs and makes appropriate referrals to services such as counseling, support groups, and other applicable services and coordinates with county workers and/or other community agencies to identify assistance funding services.
Maintains accurate and thorough records of all services provided and document all assessments, meetings, decisions, and reports.
Communicates needs, services, and progress with the referring agency, service provider, and applicants.
Provides therapeutic interventions on an ongoing basis and during crises.

Qualifications
Education and Experience:
Master's Degree from an accredited university in social work or social welfare, marriage, family and child counseling, child psychology, child development, counseling psychology, or social psychology is required.
Bilingual (Spanish), preferred.
Valid CA Driver License with a minimum of three (3) years good driving history and must be approved by the agency's insurance carrier to drive agency vehicles.

Under general supervision, investigates requests for public assistance in accordance with well-defined and established legal provisions, policies, and procedures in order to determine the eligibility and needs of applicants and recipients for public assistance and services; recommends assistance grants on basis of findings; provides other miscellaneous services; maintains various case records; and performs related duties as required.

The Behavorial Health Division is looking to fill a variety of Mental Health Specialist II vacancies across multiple programs. Mental Health Specialist II positions work collaboratively with Mental Health Clinicians, Registered Nurses, Nurse Practitioners, Mental Health Peer Support Workers, Substance Abuse Counselors, and Outreach Interventionists to provide de-escalation, stabilization, and linkage to services for adults experiencing a behavioral health crisis in the community.

Job Description
Under general supervision, investigates requests for public assistance in accordance with well-defined and established legal provisions, policies, and procedures in order to determine the eligibility and needs of applicants and recipients for public assistance and services; recommends assistance grants on basis of findings; provides other miscellaneous services; maintains various case records; and performs related duties as required.
Examples of Duties:
Connect clients to community resources, address barriers to access, and advocate to help resolve issues that limit participation in programs.
Schedule and conduct interviews to assess applicants' service needs, including in-home visits as required. Verify information received through phone and written verification, and evaluate eligibility based on collected data.
Assess client needs, create individualized service plans with goals and action steps, coordinate and monitor services, prepare reports and assessments, and provide counseling on family, financial, physical, and emotional concerns.
Maintain accurate and complete files; prepare and update correspondence and narratives; document case actions, complete required forms, and process eligibility determinations for social and ancillary services in a timely manner.
Explain policies, rules, and regulations to applicants and clients, and assist with completing required forms and reports.
Conduct orientations for community agencies; build and maintain relationships with local organizations, nonprofits, businesses, and other stakeholders to promote resource programs; facilitate and mediate case conferences between staff and service providers.
Gather and analyze data to identify trends and insights; develop evidence-based recommendations; and prepare reports.
The SocialWorker (2910) may perform other duties as assigned/required.

Qualifications
Possession of a baccalaureate degree, with major course work in social service;

Child Protective Services Workers I/II demonstrate complex social services techniques and expertise in their respective cases. Casework is generally in the areas of adoptions and protective services. The responsibilities may include authorizing the provision of services and, in some cases, directly providing such services.
Current graduate students who will graduate in the next three (3) months are strongly encouraged to apply. A letter from their graduate school which indicates that they are enrolled student in good standing and meeting all the requirements for the degree and the expected date that the degree be conferred is required for submission at the time of application. Maintains clinical and administrative records in accordance with contractual and billing requirements and submits encounter forms on a weekly basis.
Participates in weekly clinical meetings; promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
Participates in quality improvement efforts related to behavioral health and integrated models of care.
Consults informally with providers as needed.
Follows all State of California mandated reporting requirements.
May serve as supervisor for psychology or MSW interns and may provide ASW or AMFT or APCC clinical supervision.

Job Requirements
Current and valid California LCSW license with the California Board of Behavioral Sciences.
Knowledge and experience providing mental health services to disadvantaged populations.
Familiarity with evidence-based practices for the treatment of depression and other mental disorders.
Proficient in Microsoft office.

Job Description
GENERAL PURPOSE
Responsible for assisting members in meeting their expressed goals toward crisis resolution and maintaining wellness while living in the community. Additional support in areas of medication management, transportation, counseling and advocacy will be provided as needed.
DISTINGUISHING CHARACTERISTICS
This is an at-will direct service position within a program. The position is responsible for ensuring that clinical needs of members are met, and that necessary paperwork is complete and accurate.
ESSENTIAL DUTIES AND RESPONSIBILITIES - (ILLUSTRATIVE ONLY)

Assess, coordinate, and facilitate the formal and informal supports required to support Transition Aged Youth (TAY) and their families to decrease crisis events and increase movement toward life goals; works in partnership with parents who are treated as experts concerning their families.
Completes required paperwork; conducts strength/needs/biopsychosocial assessments.
Develops and participates in interdisciplinary team meetings.
Completes all tasks associated with both fidelity to the START Model, program certification of the START Model, and individual certification in the START Model.
Responsible for submitting/entering outcomes/incident data into identified database or providing this to person responsible for entering the information.
Coordinates care as needed, internally and with outside agencies, Coordinates care with system partners
With the direct input from the TAY and family, helps to enlists the involvement of formal and informal community resources to support the family.
Develop individual treatment plans for each client and empowers clients in meeting their goals; assists teams with development of service plans that reflect measurable objectives; provide group and individual therapy as indicated.
Make recommendations for improvement in clinical program. Will have input on clinical issues as part of the clinical team.
Meets regularly with clients at Turning Point sites or in the field, at job sites, homes, board and care facilities, skilled nursing facilities and other appropriate sites, transports clients and families to appointments.
Coach clients and families in developing skills necessary to meet treatment goals.
Assist in establishing client's eligibility for Medi-Cal or other benefits and advocates for continuation of benefits when appropriate.
Provide “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program; Provide prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis.
Complete paperwork as assigned in a timely manner and meets agency standard for service productivity per month; all documentation meets funder, certifying body, and TPCP standards for content and configuration.
Provides, in person, after hours, “on-call” coverage on a rotational basis to program clients.
MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES
Education, Training and Experience
A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from an accredited college or university with a Bachelor's degree and 5 years experience of Mental Health Services, or a master's degree in Social Work, Psychology, or a related field; and two (2) years of varied experience as a provider of mental health services.
Licenses; Certificates; Special Requirements:
California driver's license & current vehicle insurance/registration if driving; and,
Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles.
