Key Responsibilities:
Clinical Services
Conducts diagnostic interviews, observations and contacts the community treatment providers, forensic coordinator, probation officer, entitlement providers and family members and/or significant others.
Performs evaluation of admission materials and obtains data essential for treatment planning and documents same in a full psychosocial admission assessment according to current department guidelines.
Collaborates with treatment team members to determine whether further diagnostic studies or information are needed.
Participates in developing individual Master Treatment Plans with treatment team members and take the lead in development of a transition and discharge plan.
Coordinates the completion of all applications for discharge including housing, financial, work, and psychosocial outpatient programs.
Ensures that legal encumbrances are adequate addressed and insures timely follow up with probation and parole officers.
Attends court hearings at Circuit, District, and Family courts as needed on all islands.
Provides individual and group psychotherapy, counseling, supportive therapy, training, or other modalities of treatment in compliance with the patient's Master Treatment Plan.
Establishes and maintains a therapeutic relationship with every patient assigned to the inter-disciplinary team.
Coordinates inter-agency staff reports for patients considered appropriate for discharge.
Prepares and maintains statistical forms and reports related to patient admission, changes of legal status and discharge and documents same in patient's medical record.
Plans and coordinates appropriate discharge and aftercare for each patient on the assigned caseload and documents the plan throughout hospital episode and on the Discharge Summary.
Ensures that proper education and counseling to family, caretakers, and/or other persons significant to the patient's treatment team are provided, and documents same in-patient record.
Completes all documentation required by the hospital/department according to current guidelines within required time frames.
Provides in-patient care management documentation to include treatment goals and interventions, patient treatment plans, patient responses, and related medical data.
Maintains 30-day documentation of post discharge care received by the patient.
Provides documentation, gathers data pertinent to establishing a case for court hearing, identifying key witnesses of the dangerousness displayed and determining the patient's mental condition, stability, and post hospital plans.
Assists the Attorney General's Office and/or criminal court staff in preparing an individual case for hearing as appropriate.
Testifies in court case hearings on or off Hawaii State Hospital (HSH) property regarding clarification of issues of appropriate restrictive facilities or as an eyewitness to the element of dangerousness.
Education and Experience:

SUMMARY OF DUTIES
The Family Service Worker (FSW) provides transportation and supervised visitation services in a safe setting between parents and their children in foster care. Provide transportation services to participants for case related activities to including medical appointments, therapy appointments, court hearings, unsupervised visits, or for any other case related activities that do not require monitoring or supervision. Provides childcare during the times participants are receiving CCSS related services as a way to address participation barriers. The FSW maintains participant confidentiality at all times, participates in program staff meetings, meets with supervisor on a regular basis and attends training and other staff development activities. The position maintains case recording, mileage logs and other miscellaneous reports in a timely fashion.
EDUCATION AND TRAINING REQUIREMENTS
Up to and including high school. Requires enough basic education to understand and follow standard practice or oral and written instructions, able to read and write, use simple arithmetic, keep simple records, or use simple office machines like a typewriter.

Payday

Program Mission
To provide individuals and families outreach services that build capacity, reduce environmental and social risk factors while increasing protective factors and access to resources.
Target Population
Individuals and families eligible for SNAP benefits.
I. CORE FUNCTIONS
Administrative Duties
Welcomes visitors and callers to identify needs
Takes messages and forwards to staff
Retrieves and sends mail from the main office for processing

Perform general clerical duties such as word processing, correspondence, e-mail, telephone and in-person reception, filing, data entry, record maintenance, faxing, scanning and timely and accurate data entry.
Program Duties
Provides both direct and outreach services to the persons served as a part of Wai‘anae Neighborhood Place.
Respects the cultural values and beliefs of persons served.
Assists with the intake/application process with the persons served in completing demographic and authorization, consents and other necessary information.
Conduct outreach activities to reach the target population, including, but not limited to, canvassing the community, distribution educational material, making public presentations, and staffing resource fair booths and tabling.
Completes weekly, monthly and annual reporting responsibilities.
Provides referrals to any applicable resources, or any other appropriate State agency or assistance programs, and maintains a list of, and establishes a protocol for, referral services available to assist the persons served with questions, grievances, complaints or questions about assistance and/or services.
Supports the program's quality activities as well as the Agency's Performance and Quality Improvement Plan.
Operates, as needed, the Donation Closet
Provides both direct and outreach with the person served, as needed, when the assigned worker is not available.
Provide outreach, education and enrollment assistance to individuals and families potentially eligible for SNAP assistance.
Assist with completing SNAP applications, securing and maintaining benefits and authorizations for information sharing.
Maintaining expertise in pre-screening, application, and program specifications.
Conduct education activities to community groups, including, but not limited to workshops, brown-bags, and one-on-one direct services.
Provide assistance without imposing a fee, and assist all applicants or beneficiaries, regardless of age, disability, or culture.
Provide information in a manner that is culturally and linguistically appropriate to the needs of the population being served, including individuals with limited English proficiency, and ensure accessibility and usability of tools and functions for consumers with disabilities in accordance with the Americans with Disabilities Act and section 504 of the Rehabilitation Act.
Help the person served to make an informed decision by providing information and services in a fair, accurate and impartial/unbiased manner.
Answer questions to facilitate pre-screening and document readiness.
Inform applicants that the Department of Human Services is solely responsible for making eligibility determinations.
Enter all contacts, call logs, requests, demographic profiles and reports on DHS approved forms.
Participate in monthly provider meetings with DHS SNAP personnel, and other SNAP outreach providers.
