Salary Schedule
To provide school social work services and referrals to students and families that will meet the social and emotional needs of students and promote a successful learning capacity for all students. To work in collaboration with student services personnel, both district and school-based.
Responsibilities and Qualifications
QUALIFICATIONS:
Master's degree in social work from an accredited college or university.
Certification or eligible for certification by the Florida State Department of Education.
Must have and maintain a valid state of Florida driver's license and/or have access to transportation.
KNOWLEDGE, SKILLS AND ABILITIES:
Ability to communicate effectively using written and oral communication skills; ability to use effective positive interpersonal skills; knowledge of state and federal laws related to school attendance, child abuse and HIPPA; knowledge of community resources and community contacts; knowledge of and the ability to interact with individual, group and family dynamics; ability to analyze reports, social histories and behavior assessments; commitment to a core set of beliefs about learning, teaching and ongoing professional learning; knowledge of effective social work theories and practices; ability to provide effective social work practices; and ability to make sound decisions.

JOB DUTIES AND RESPONSIBILITIES:
* Manifests a professional code of ethics and values
* Responds to internal and external customers in a timely, accurate, courteous and empathetic manner representing OCPS in a positive light
* Model the routine, intentional and effective use of technology in daily work, including communications, organization and management tasks.
* Collaborate and meet regularly with the school's Student Services Team to support all tiers of mental health support and resiliency initiatives.
* Participate in and support the multi-tiered system of supports (MTSS) process to assist with addressing student needs in all areas (i.e., attendance, academic, behavioral, social, health, and mental health, etc.) This includes but is not limited to MTSS, ACST, IEP, 504, and discipline meetings, as appropriate
* Serve as a representative at appropriate meetings and activities pertaining to the delivery of school social work services and ensure that federal, state, and district requirements are met.
* Conduct assigned portions of evaluations of students for ESE services in accordance with social work areas of responsibility
* Provide instructional lessons on resiliency topics such as Child Safety Matters and lessons following specific disciplinary infractions.
* Provide direct individual and group counseling services (this may include counseling as a related service on Individual Evaluation and 504 plans)
* Support Threat Management Teams and serve in an appropriate capacity as defined by the district
* Support students who exhibit self-harm, or suicidal ideation and ensure appropriate interventions are implemented
* Collaborate with school-based attendance team to support all tiers of support related to attendance and truancy
* Connect families with community resources and information to meet student and family needs
* Conducts home visits for the purpose of assessing areas that may interfere with learning and/or appropriate educational placement
* Support district crisis response
* Provide timely documentation through district tracking and accountability systems (digital and cumulative) as part of their job responsibilities

Must have, or be in the process of acquiring, certification from the Academy of Certified SocialWorkers.
Two years' experience preferred, with at least one year of experience in a healthcare setting (hospital, clinic, rehabilitation center, etc.).
Must have a criminal Level II background clearance.

POSITION SUMMARY
To improve the health status of our market's medically underserved population by providing superior quality, competitive value and outstanding service and care in mental health. This position is responsible for providing patient care in mental health and initiating the process for successfully integrated mental health services.
GENERAL EDUCATION REQUIREMENTS
Current supervision (post-master's degree) by an included BHA appointed individual who has obtained a Master's Degree in social work from a Council on Social Work Education (CSWE) accredited school of Social Work. See Florida Statute Chapter 456: Health Professions and Occupations.
ADDITIONAL QUALIFICATIONS
Experience in the clinical operational management of community health centers
Proficient in Electronic Medical Records (EMR)
Proficient in the operations of Federally Qualified Health Centers (FQHC)
Excellent verbal and written communication skills
Excellent collaboration skills required for community relationship building
Automated office and PC experience required
Current BLS/CPR Certification
Basic knowledge of psycho-pharmacology
Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance
Ability to design and implement clinical pathways and protocols for treatment of selected chronic mental and behavioral conditions
Ability to make quick and accurate clinical assessments of mental and behavioral conditions
PRIMARY ACCOUNTABILITIES
Maintain predetermined productivity of individual and group interventions.
Consult with patients and co-manage the treatment of mental disorders and psychosocial issues.
Responsible for initial patient contact, coordinating referrals, assessments and developing treatment plans.
Interview patients, give diagnostic tests, review records, conduct assessments, and confer with other professionals to diagnose disorders and identify and evaluate psychological, emotional, or behavioral issues.
Develop and implement behavior modification programs and/or treatment and intervention programs that patients can understand and comply with.
Monitor, evaluate, and record patient progress according to measurable goals described in treatment and care plan. Modify treatment plans to comply with changes in patients' status.
Provide therapy and consultation to patients in individual and group sessions to assist them in dealing with substance abuse, mental and physical illness, poverty, unemployment, or physical abuse.
Collaborate with Providers, Case Managers and the Nursing team to plan and coordinate treatment, drawing on social work experience and patient needs.
Educate patients and community members about mental and physical illness, abuse, medication, and available community resources.
Maintain accurate records of treatment in the patient Electronic Medical Record (EMR) and follow up with patients in order to evaluate the effectiveness of counseling or treatments.

Roles and Responsibilities
Perform in-home and telehealth care management visits to assess and determine social and behavioral status

Position Requirements
This position involves telephonic visits with some car travel to patients' homes
Basic Life Support (BLS) certification is required in this role. The company will support your certification completion through onboarding.
Currently licensed as a LCSW or LMSW in the posted state
Master's degree in social work and passed ASWB masters or clinical exam
Rare domestic travel may be required to Brentwood, TN
Self-starter with the ability to work independently with minimal supervision
Ability to show empathy and quickly build relationships with patients and local CBOs
Preferred 2+ years previous experience working in care management and/or with chronic illness
Excellent verbal communication skills both in person and on the phone

Two years' experience preferred, with at least one year of experience in a healthcare setting (hospital, clinic, rehabilitation center, etc.).
Must have a criminal Level II background clearance.
Must have current CPR certification.

The MSW is responsible for optimal patient flow/throughput to enhance Continuity of Care, smooth and safe transitions, patient satisfaction, patient safety, readmission prevention and length of stay management. The Clinical SocialWorker communicates daily with the interdisciplinary team during daily multidisciplinary rounds. Care coordination, discharge planning, transitions of care planning and are core competencies. The MSW facilitates the collaborative management of patient care across the continuum.
The MSW intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources and qualify for community assistance from a variety of special assistance programs and agencies, and/or require assistance with Transitions of Care or Discharge Planning.

Qualifications:

Care Management experience
Knowledge of state and federal guidelines pertinent to Medical Case Management

Harbor House of Central Florida is Orange County's state-certified Domestic Violence service provider. We are dedicated to supporting survivors of domestic violence through comprehensive services, including a 24-hour crisis hotline, emergency shelter, counseling, legal advocacy, and community education.
Position Overview: The Court Services Advocate provides advocacy services to survivors of domestic violence who are attempting to secure injunctions for protection by providing crisis intervention, advocacy, and referrals.
Key Responsibilities:
Assist survivors seeking domestic violence injunctions at the Orange County courthouse by providing referrals for supportive services to other Harbor House programs, as well as other community agencies. These referrals may include counseling, support groups, victim's compensation, etc.
Provides immediate support, crisis intervention counseling and support groups to survivors of domestic violence.
Provides front-desk coverage as needed including answering phones, greeting clients, disseminating and receiving information and documentation from clients, and ensuing clients are seen in a timely manner.
Provides support for survivors at court hearings.
Provide support and guidance to survivors who are filing petitions for injunction at the clerk's office computer room and those seeking assistance in the waiting area
Explain injunction orders to survivors.
Explain the procedure and purpose of injunction.
Assists survivors in making plans to return for court hearings and discusses what to courtroom expectations.
Provides immediate support and crisis intervention counseling to survivors whose partners are being prosecuted by the State Attorney's Office.
Establish and maintain partnerships within OC Courthouse and other community-based organizations, by engaging in periodic tabling events, meetings, coalitions and other community events.
Works closely with Harbor House Shelter Services Advocates to ensure coordination of services.
Records and compile statistical data on clients and services that will be submitted to the Outreach Services Manager.
Qualifications:
Education: Minimum of an undergraduate degree in Social Work, Criminal Justice, Counseling, or related field is preferred.
Experience: A minimum of one year in counseling or social work experience, preferably in a domestic violence program or criminal justice field.
Discretion: Professional experience may be substituted for formal education at the discretion of the Chief Executive Officer.
Licensing: Valid Florida Driver's License with a clean record for the past 5 years. Must be 21 years or older to meet agency insurance requirements.
Skills: Proficiency in Microsoft Office applications (Word, Excel, PowerPoint). Ability to learn organization-specific software, including databases and content management systems. Strong professional writing and communication skills. Ability to manage multiple priorities in a fast-paced environment with attention to detail. Knowledge of case management, and the ability to provide effective and immediate crisis intervention.
Training Requirements: Completion of CORE Competency training within 90 days of hire. Additional mandatory training is required on an annual basis per Agency Policies & Procedures.
