5. Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis.
6. Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources.
7. Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual's needs.
8. Act as liaison with the program after hours staff on a daily basis through exchange of participant information; briefly detailing the presenting problem and after hours plans for the individuals being served by the Assertive Community Treatment Team.
9. Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or pager. Meet regularly to review the total caseload, problem-solve, develop plans, and determine daily duty assignments.
10. Complete an intake assessment including: medical history, general physical condition, alcohol and drug histories and current use, educational issues, family structure and relationships, vocational history and goals, legal history and issues, psychiatric history (including, onset, course, effect of illness, treatment history, medication history risk behaviors).
11. Ensure immediate and appropriate interventions are provided in response to changes in mental status which may put individual at risk.
12. Documents participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes.
13. Developing service plans and/or person-centered plans as appropriate, and screens referrals in coordination with the other Assertive Community Treatment Team members.
14. Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
15. Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
16. Follow service definition guidelines for services being provided.

Our client is seeking a full-time Counselor for our client's Women's Residential Primary Care Program. Requires at least three years of experience in addiction and integrated co-occurring counseling. All documentation must be completed by the end of each shift worked.
7. Submit all required documentation by the deadlines specified by the Supervisor.8. Obtain and keep current all training required by the agency, according to the service provided and according to the individual's needs. 9. Attend all mandatory staff meetings and consumer conferences or give advance notice to the Supervisor as to reason(s) for inability to attend.10. Receive supervision from a Supervisor/Qualified Professional (QP) at least quarterly and as needed either face-to-face, via phone, or group staff meetings. All paraprofessionals and associate professionals will be supervised by a Qualified Professional in accordance with G.S. NCAC 27 G .0204 (b) (c) (f) and according to licensure or certification requirements of the appropriate discipline.11. Conduct intake and assessments for residents for enrollment into the case management program.
Conduct home visits, office visits, community site visits to collect information from clients.
Work with families to assess, develop, implement, monitor, and recommend modifications to Development Plans using a multidisciplinary process.

This Opportunity:The primary responsibilities of the Assertive Community Treatment Team Substance Abuse Professional are to assist and support individuals who are served in the development of skills, behaviors, responsibilities, and supports needed to live, learn, work and be integrated into the community with the greatest possible degree of self-determination and independence.What You'll Do:
• Provide service coordination for individual with mental health and substance abuse issues and is responsible for developing, with the other team members, the overall service plan.
• Assist in the development of natural support networks. Individual and group supportive counseling, monitoring of symptoms and medications, linking the individual supported to the appropriate services, agencies and supports in the community, monitoring their development of appropriate behaviors in his/her home and other community settings, teaching the individual supported skills necessary for independent community living and facilitating them in the practice and transfer of these skills to his/her environment, providing support to his/her family.
• Assume responsibility for writing, implementing, evaluating, and revising Person Centered Plan and overall treatment goals and service plans that are based on evaluations and recommendations from assessments by various professionals and input from the individual supported.
• Function as a member of a multi-disciplinary team to develop and implement treatment goals.
• Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis.
• Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources.
• Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual's needs.
• Act as liaison with the program after hours staff on a daily basis through exchange of participant information; briefly detailing the presenting problem and after hours plans for the individuals being served by the Assertive Community Treatment Team.
• Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or pager. Meet regularly to review the total caseload, problem-solve, develop plans, and determine daily duty assignments.
• Complete an intake assessment including: medical history, general physical condition, alcohol and drug histories and current use, educational issues, family structure and relationships, vocational history and goals, legal history and issues, psychiatric history (including, onset, course, effect of illness, treatment history, medication history risk behaviors).
• Ensure immediate and appropriate interventions are provided in response to changes in mental status which may put individual at risk.
• Documents participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes.
• Developing service plans and/or person-centered plans as appropriate, and screens referrals in coordination with the other Assertive Community Treatment Team members.
• Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
• Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
• Follow service definition guidelines for services being provided.
• Complete all other relevant responsibilities as assigned by the supervisor.
• Driving and travel may be required.
