    Full-time Description Professional Medical Practice, P.C., is looking for an Alcohol and Substance Abuse Counselor (CASAC or CASAC2) to work in the medical departments at both the Montgomery County Jail in Fultonville, NY and the Fulton County Correctional Facility in Johnstown, NY This is a Full-Time position, working 40 hours per week (Monday-Friday) between the Montgomery County Jail and the Fulton County Correctional Facility. Hourly Rate: $24.00- 28.00. You must have a CASAC or CASAC2 to qualify for this role. Job Responsibilities: Responsible for the clinical monitoring, coordination, and implementation of the behavioral health program in accordance with Company policies and procedures. Serve as the liaison between medical and facility staff as it pertains to Medication Assisted Treatment (MAT). Engage patient in therapeutic interventions with the goal of addressing substance use related concerns. Facilitate group and individual sessions with patients in the MAT program. Actively assist with the detection and prevention of patient suicide. Document all patient encounters in a timely manner utilizing the SOAPE note format. Assist and/or collaborate with facility staff regarding discharge planning for patients in the MAT program, where applicable. Facilitate/participate in interdisciplinary team meetings and/or case consultations with staff across disciplines. Participate as a member of the Quality Assurance Committee and any sub-committees. Assist in the establishment of Quality Assurance standards and audit criteria. Serve as a member of various committees within the medical unit and facility as necessary. Services available to residents include crisis intervention, rehabilitation counseling, social and living skills development (money management, personal hygiene, food preparation, meal planning, recreation activities, etc.), symptom and medication management, community integration and resource development. On duty around the clock staff work closely with each resident to maximize stability and help in the transition to a less restrictive environment. Longer term assistance is available for people with problems preventing them from living outside a group setting. Community Living Apartments The Community Living Apartments Program offers two types of living arrangements: Maple Street and Satellite apartments. Both include support and guidance through individualized service plans created in partnership with the service recipient. Services are a continuation and expansion of those services provided in the Community Residences and carry the ultimate goal of achieving an independent, successful life for each resident. Residents learn more about managing symptoms through medication and therapy, improving daily living skills, pursuing educations, vocational and employment goals, solving transportation needs, and increasing their comfort with broader social interaction. Staff meet with recipients from one to seven days each week to provide direct services and supportive counseling.     The Prevention Family Specialist goal is to preserve the family unit by providing supportive casework counseling in the home and empower parents with tools and resources to ensure children's safety, well-being and permanency while providing advocacy to ensure quality services. Qualifications Job Responsibilities Conducts assessment of family's strengths and needs; assesses problem areas and utilizes various treatment modalities and resources to effect growth and change. Participates in development, implementation and ongoing evaluation of comprehensive treatment and discharge plan. Develops alternative placement or permanency resources for youth that may not return to their families. Completes a comprehensive discharge summary at least eight weeks prior to discharge. Delivers effective case management services, including ensuring counseling sessions are scheduled as needed and children receive required medical and dental examinations. Tracks and monitors services provided, completes all documentation in a timely and comprehensive manner. Ensures compliance with confidentiality standards, parental consent and necessary release of information. Conducts casework contacts with families, as needed but at least twice a month, in their homes and community settings to focus on treatment goals. Engages youth and family by developing effective and caring relationship. Ensures children are maintained in safe, secure environment. Develops and maintains relationships with community schools to ensure the best educational experience for each child and with community resource agencies.
