The CEM builds a caseload from their desk focused on consistently delivering expected services, as demonstrated by their ability to meet or exceed all established metrics. Utilizes Thrive SPC's applicant tracking system for candidate management. Works with Talent Acquisition Team to schedule interviews. Interviews and hires direct care team members. Engages with candidate through onboarding process to maintain relationship. Assists the Onboarding Specialist, when necessary, with any onboarding requirements. Coordinates the opening of new referrals in conjunction with the admitting clinician. Utilizes strategic staffing methods to optimize the care coverage for all patients and direct care team members, including communication of all schedule changes, ensuring patients are staffed to meet the patient/guardian's needs with an appropriately skilled clinician, and within insurance authorization. Responsible to manage utilization of direct care team members based on patient needs as well as the desired hours of the direct care team member. Prepares and distributes the patient/direct care team member schedule. Performs initial review of schedules for productivity/geographic issues. Completes daily and weekly pre-bill audits to ensure proper payor utilization and appropriate staffing per payor authorizations, and processes payroll. Responsible for performance management of direct team members, including annual performance reviews. Retain and engage direct care team members by creating an employer of choice experience. Proactively communicates with clients and direct care team members to discuss needs and/or concerns. Documents communications per Thrive SPC's SOP. Informs location director or designee of emergency or problem situations, especially clinical, safety, or compliance concerns. Manages the annual recredentialing process. Process all transactions related to employee status changes and terminations. Responsible to ensure personal files are up to date and accurate for direct care team members. Distribute and compile satisfaction surveys sent to clients, physician, and employees. Maintain appropriate level of location/office supplies and order as needed. Conduct home and/or hospital visits with Clinical Supervisors and or Area Manager as needed. Completes location reports as needed. Navigate Thrive SPC's EMR system in a proficient and effective manner. Participates in on-call rotation/after-hours support. Assists with all incoming calls; taking messages, routing appropriately, and addressing client/direct care team member needs as per Thrive SPC Policy and Procedure manual.     Thrive Skilled Pediatric Care's purpose is simple, to provide high quality clinical home care to medically fragile children so they can grow and flourish to their full potential. The Speech and Language Pathologist (SLP) is a licensed graduate of an approved SLP school and is licensed by the state in which they practice. SLPs administer skilled care to clients requiring intermittent professional services and teach the client, family and other members of the health care team. These services are performed in accordance with the physician's orders and the established plan of care, under the direction and supervision of the Therapy Program Supervisor or Clinical Director. HIRING REQUIREMENTS Must have graduated from an accredited SLP school and licensed in the state in which he/she currently practices. SLPs must possess their Certificate of Clinical Competency, but will consider CFY if a preceptor is available Current CPR certification At least one-year experience in physical, occupational or speech therapy (pediatric preferred, not required) Must possess a valid driver's license and required automobile insurance. ESSENTIAL RESPONSIBILITIES Perform client visits in a timely, professional, and appropriate manner per standards of care. Ensure in-home visit time fully meets the needs of the patient. Complete all appropriate documentation and submit timely per protocols, policies and procedures. Contact physician regarding patient changes, receive and write verbal orders. Review and verify schedules weekly to ensure all scheduled visits are accurate. Ensure all scheduled procedures are documented properly and appropriately. Update the Plan of Treatment, notify the physician and designated agency office staff of significant changes in the patient's status. Complete any required documentation on approved systems and/or forms. Report the need for add-on services to the appropriate location team members. For clients not receiving PDN services, review and update the Medication Profile by listing medications and dosages as labeled. Cooperate and assist other healthcare-related agencies providing or assuming health care services to enhance continuity of care. Complete admission procedures according to Thrive SPC's established procedures governing admissions. Complete re-certification and follow up assessment procedures according to Thrive SPC's established procedures governing re-certifications and follow up assessments. Complete post hospital and follow-up assessments according to Thrive SPC's established procedures governing post hospital and follow-up assessments. Conduct discharge assessments and ensure appropriate discharge teaching to enhance client satisfaction. Retrieve the Home Chart if discharging the patient from the agency. Plan, coordinate and perform all patient functional re-assessment visits per Medicaid/Insurance regulations. Ensure that missed patient visits are documented and re-scheduled within allowable timelines to adhere to the patient's plan of treatment. Participate in staff development meetings and in-services. Participate in and document case conferencing and coordination of care ensuring appropriate care and consideration of need for continuation or discharge. Participate in planning, operation and evaluation of therapy / nursing services. Adhere to and participate in the Agency's mandatory HIPAA/Privacy Program and Employee Compliance Program. Provide/conduct supervisory visits in a timely manner as required for Speech and Language Pathology Assistants as to ensure appropriate service provision and compliance to all State and Federal regulations. Obtain continuing education to improve care by broadening knowledge of changes in health care delivery and to meet any licensure requirements. All other duties as assigned.
