LVN License required
3-7 years of experience in facility/physician relationships with a deep understanding of facility discharge processes, preferred.
Minimum of one year experience in home care, hospice, or personal care services, preferred
Prior use of Electronic Medical Records (EMR), preferred
Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred.
Responsibilities:
Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
Obtains & Documents Verbal Orders for patient evaluation, as required in state in which the Territory Patient Admission Coordinator is licensed.
Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency's software solution to facilitate the referral process by completing required workflow task within the Company's established time frames.
Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.

Must have driver's license and able to travel locally to physician and facility offices to obtain order signatures and requirements.
Responsibilities:
As a Community Liaison, you will:
Process and track physician orders.
Deliver orders to doctors and coordinate with their offices.
Follow up on outstanding orders and resolve any issues.
Assist with obtaining necessary documentation from external sources.
Complete the process for signed orders, including entering them into the electronic medical records system.
Communicate with other staff about the status of orders.

Participates in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.
Supervise paraprofessional staff, as assigned.
Participates in quality and performance improvement measures.
Participates in in services.
Actively communicates with the supervising physical therapist, as well as other members of the multidisciplinary healthcare team providing care to the patient in order to promote coordination of care.
Complete other assignments as requested and assigned.
Access use and ability to disclose personal health information (“PHI”) necessary to fulfill the above duties and responsibilities and as defined by each organization.
Licensure, Minimum Education & Experience Requirements:
Licensed, registered or certified as a physical therapist Assistant by the state(s) in which he or she will provide services, as applicable per the state requirements.
Graduate of 2 year college-level program approved by the APTA or CAPTE.
In the last five (5) years, two (2) years of experience as a PTA that is relevant to the patient population assigned
Provide proof of valid driver's license, as per organization-specific policy.
Provide proof of Physical Therapist Assistant License
