Full-time, Part-time Description The Rehabilitation Counselor (RC) is responsible for organizing services and support to facilitate a client's recovery and achievement of life goals to improve functioning in the home, school or workplace, and community. Requirements ESSENTIAL DUTIES AND RESPONSIBILITIES: Meet with clients regularly and consistently to work on client goals (six times a month for adults, four times a month for minors). Develop and implement Individual Rehabilitation Plans (IRPs) with clients to outline goals for recovery. Develop and implement initial, monthly, and annual individualized rehabilitation assessments to evaluate client needs and progress. Conduct regular in-home or community based client visits to provide individualized case management, counseling, coaching, and skills training as needed. Link clients to government and community resources as needed, and assist in clients obtaining those resources. Ensure client maintenance of physical and mental health and well-being. Support client cooperation with treatment recommendations from other providers (therapy, medication management, medical appointments, etc.), and maintain communication with those providers.
