WHAT WILL YOU DO
This individual is responsible for accessioning and preparing specimens for gross examination, maintaining logs and preparing specimens for transport to the histology laboratory. He/she may perform routine and limited special stains for immediate diagnosis. Other duties include weighing and measuring specimens, assisting with the gross, embedding specimens, operating the tissue processor and cutting. He/she will also assist with fine needle aspirates, biopsies, frozen sections, autopsies and cytological specimen preparation and staining to provide data in the functioning of tissue and organs, causes and/or progress of disease. Clerical duties include completing histology computer work, gathering workload data as requested by supervisor and maintaining adequate inventory of supplies as indicated by supervisor. WHAT WILL YOU DO
This individual is responsible for collection of blood specimens, for performing laboratory office duties, and processing laboratory specimens. Laboratory office duties will include registering patients, accurately and completely entering all necessary data into the computer system, specimen accessioning, answering telephone calls, and processing paperwork. Specimen processing duties will include preparation of specimens for testing and/or for sending to the central laboratory or to a reference laboratory. In addition to handling blood, body fluid and urine specimens according to established procedures, they may also perform limited waived and specified non-waived POC tests as authorized by the Laboratory Director. In some facilities this individual, as required, will act as a driver and participate in the on-call rotation.

PURPOSE AND SCOPE:
The professional registered nurse Inpatient Services RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the kidney disease health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and program operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care, policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations.
· Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse.
· Performs ongoing, systematic collection and analysis of patient data pre - during - post treatment for assigned patients and documents in the patient medical record, makes adjustments and modifications to treatment plan as indicated and notifies Supervisor, Physician, patient's primary nurse and others as may be indicated.
· Assesses, collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.

· Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
· May be asked to provide specialized nursing care instruction to hospital/facility staff as stipulated contractually.
· May be assigned to assist in an Outpatient facility on an as needed basis.
· Required to complete CAP requirements to advance.

PRIMARY RESPONSIBILITIES AND DUTIES:
Performs all job duties and responsibilities of a Special Procedures and Cardiac Cath Technologist. Performs diagnostic procedures according to established policy and procedure.
Administers IV contrast media in accordance with department policy and procedure.
Ensures that proper sterile techniques are applied appropriately to prevent cross contamination.
Works directly with Radiologist in determining protocol for all special procedure imaging.
Works directly with Cardiologists to ensure proper coordination and delivery of Cardiac Cath Services.
Explains procedure to patient prior to procedure start. Provides patient and family with education for both pre and post procedure.
Observes patient for behavioral and physiologic changes due to medications. Notifies physician and Nurse with any status change.
Assists with the operation and handling of special equipment, monitors, and instruments.
Demonstrates the ability to work as a team membe, assists physicians and assists on all levels of patient care inside the Lab. This includes but not limited to circulator, scrub, transfers, patient escort, transportation of specimens to appropriate department, scheduling, ordering of supplies, etc.
Observes and documents patient data and procedure record throughout procedure with appropriate hemodynamics system. Records heart rate, BP, respirations, all pressures, O2 saturations, and EKG. Recognizes EKG rhythms and disrhythmia.
Ensures patient and own safety by following all applicable radiation protection measures and requirements. Maintains safety regulations to ensure maximum protection to patients, staff and employees of hospital.
Maintains QC on equipment. Responsible for operation and safety of all equipment. Reports all equipment problems in timely fashion.
Demonstrates effective business conduct through behavior/performance example and by following HCH Performance Standards.
Maintains patient confidentiality and follows all applicable HIPAA standards.
Maintains professional licensure and personal education through professional affiliations, meetings and educational seminars.

As a Hospice Registered Nurse Case Manager (RN), you will:
Oversee the implementation and ongoing assessment of the patient's plan of care alongside the spiritual care coordinator, social worker, hospice aide and medical director
Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs
Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve
To qualify for a Hospice Registered Nurse Case Manager (RN) with us, you will need:
Education: Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required; Bachelor of Science in Nursing (BSN) preferred
Licensure: Current license to practice as a Registered Nurse (RN) in the state associated with this position.
Current CPR/FA certification
Reliable transportation to/from care sites and/or work locations
One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting preferred

The RHS Nurse is primarily responsible for delivering care coordination and case management services for residents and families, as well as providing ongoing education for our Independent Living Residents. This is a full-time, hourly position onsite in Wheaton, IL within our Independent Living facility.
Here are a few of the daily responsibilities of an RHS Nurse:
Evaluates the physical, emotional, and social needs of residents
Maintains or improves independence through directing and educating residents, advising on appropriate adaptive equipment use
Assess resident's cognitive and physical abilities and recommends adaptation to living environment to reduce barriers of safety and well being
Assists in the orientation of new residents' adjustment to community living
Maintains appropriate and complete reports, records, physicians orders, care plans, and progress notes for residents. Assures confidentiality of resident information
Maintains communication with physicians, families, and appropriate staff regarding resident status
Assists residents and families by
Offering support in dealing logistically and emotionally with the aging process and illness
Provides accurate information and timely updates to residents and families
Addresses questions and concerns promptly, or identifies appropriate parties/resources to do so
Assist and stimulates residents to participate in social and creative activities
Assists in coordinating services for resident being admitted to and discharged from Assisted Living, Health Center, and Hospital
Monitors infections and accidents of residents and notifies families and physicians when appropriate
Monitors private duty personnel in accordance with community policies and procedure, coordinates care with agency representatives
Responds with urgency to emergent health issues of our residents
Here are a few of the qualifications we need you to have:
Minimum two years clinical or case management experience with elderly population strongly preferred
Current Registered Nurse or Licensed Practical Nurse with active license by State of Illinois
Current CPR/BLS certificate
Comprehensive understanding of the English Language; must be able to read, write, and communicate in English
Physical Requirements
Able to walk long distances 50% of the work day
Must be able to lift and carry up to 30 lbs
Must be able to push a wheelchair occupied by a resident weighing up to 200 lbs. independently
Must be able to safely respond to emergent situations throughout the entire Independent Living building
