Responsibilities
* Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
* Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.

Qualifications

* • Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.
* Submits appropriate documentation and paperwork to facilities at the completion of patient visits; documentation is completed and submitted timely according to company policy.
* Participates as a member of the IDT, including development and implementation of the plan of care.
* Provides psycho-social support and counseling services to the patient and family; supports the involvement of external counseling resources commensurate with patient/family needs and scope of practice.
* Assists in identifying the need for intervention of other IDT members.
* Effectively communicates patient and family needs to IDT.
* Collaborates with IDT to coordinate psycho-social care and support for the patient and family to ensure appropriateness, continuity, and quality of care.
* Maintains updated, comprehensive care plans which reflect current problems, goals and interventions for patients.
* Maintains respect for the family's environment and belief systems, and remains nondiscriminatory regarding age, race, religion, sex, sexual orientation, national origin, physical or mental disability, and other personal matters.
* Completes initial and ongoing psycho-social assessments for patients; identifies and addresses concerns/issues related to financial means, insurance, living arrangements/placement for long-term care, caregiver stressors, pre-bereavement and anticipatory grief, and community resource needs.

* Required - Minimum of Bachelor's degree in Social Work, Psychology, Sociology, or other field related to social work with one (1) year of social work experience in a healthcare setting required; and licensure if required by state rules. * Preferred - Master's degree in Social Work with one (1) year of social work experience in a healthcare setting highly preferred; and licensure if required by state rules.

* BSW from an accredited school of social work is minimum education required but must be supervised by MSW.

Minimum Qualifications for a Hospice Physician:
Licensed to practice medicine or osteopathy without restriction in the state of Missouri. Proof of current licensure will be required
Graduate of an accredited and approved medical program as required, based on Missouri state regulations
Board certification in internal medicine, family medicine, emergency medicine preferred
Specialty board certification in hospice and palliative care preferred
Minimum of two (2) years of recent, verifiable, related clinical experience, preferably at least one (1) year in hospice, palliative and/or home care
Must maintain a valid registration with the DEA and any applicable state agency without limitation to prescribe drugs under such registration. Will provide proof of such registration upon request
Must remain a qualified Medicare and Medicaid provider
Will maintain unrestricted medical staff membership and clinical admitting privileges at any of Hospice's inpatient contracts

Job DescriptionBAYADA STATE Hospice has an immediate opening for a Part Time Spiritual Counselor. This Hospice Spiritual Counselor position is for visits in the Lebanon, MO and surrounding area. This individual will be a member of an experienced hospice care team that provides assessments, education and symptom management related to end of life care. Hospice / care experience preferred, along with experience making home visits.

Responsibilities for a Hospice Spiritual Counselor include:
Develop and adhere to the client's spiritual care plan.
Provide spiritual and emotional support to patients and family members.
Complete documentation accurately and timely.
Must be reliable and punctual.
Must demonstrate an appropriate attitude.
Participate in providing bereavement services to family members, referral sources, and the community.
Qualifications for a Hospice Spiritual Counselor include:
Bachelor's degree required, Master's degree in Counseling preferred or have graduated from an approved divinity school.
One year of experience in a spiritual counseling capacity in a health care setting, preferably hospice or palliative care.
Formal ordination or endorsement by a recognized faith organization is preferred, or, for faith traditions that do
not require ordination, demonstrated spiritual leadership, or recognized standing within the community is
preferred.
Must be willing to travel coverage area of office.

What you'll do:
Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.
Responsibilities for a PT - Physical Therapist include:
Make home visits to clients in designated geographic territories.
Focus on Orthopedic patient visits.
Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
What you should have:
Active and current PT license
One year of clinical experience preferred
Previous home health care experience welcomed, but not required. We will train you!
Ability to travel locally to cases as assigned
