This office services adult and geriatric clients on a per visit basis in territories throughout the Pitt County area.

Responsibilities for a PT - Physical Therapist include:
* Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
Qualifications for a PT - Physical Therapist include:
A current North Carolina Physical Therapist - PT license.
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.

BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist, SLP, for a per diem opportunity performing home health visits for our office located in Greenville, North Carolina.
This office services adult and geriatric patients in homes throughout Pitt, Craven, and Beaufort counties.
Ideal candidate will have 1-2 days per week availability and willing to travel to all counties.
Prior home care experience as a Speech Language Pathologist, SLP is a plus, but not required.
As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Speech Language Pathologist, SLP Responsibilities:
Make home visits to clients in designated geographic territories.
Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.
Develop a treatment plan based on the evaluation, physician's orders, and recommended goals.
Continually assess and revise the speech therapy care plan, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.
Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet.
Speech Language Pathologist, SLP Qualifications:
A current license as a Speech Language Pathologist in North Carolina.
Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or;
Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification, or;
Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.

A nurse aide training and competency evaluation program approved by State and meets the requirements of the federal regulations (§483.151 through §483.154) and is currently listed in good standing on the state nurse aide registry;

The requirements of a state licensure program that meets training and competency evaluation provisions per State and the federal regulations (42 CFR §484.80) AND
Has not had a lapse in work for compensation for the period of 24 consecutive months as a home health aide or nurse aide.
If there has been a 24-month lapse in furnishing services for compensation, the individual must successfully complete another program, as specified above in this section, before providing services.
Knowledge, Skills & Abilities Required:

Evidence of passing a home health aide written knowledge exam.
Pass state required criminal and abuse background checks where required.
Able to effectively communicate in English with all levels of the workforce, both clinical and non-clinical

**Qualifications:**

We are currently seeking a dedicated Clinical Manager within our skilled nursing pediatric/adult home health care team to lead our clinical team and ensure excellence in care delivery. Our main office is located in Greenville, NC.
Position Summary:
The Clinical Manager oversees the day-to-day clinical operations of pediatric home care services. This role ensures that all care provided meets clinical standards, regulatory requirements, and organizational goals. The Clinical Manager will be responsible for supervising nursing staff, managing care plans, collaborating with interdisciplinary teams, and maintaining strong relationships with patients and their families.
Key Responsibilities:
* Lead and supervise a team of registered nurses (RNs), licensed practical nurses (LPNs), and other field staff, ensuring high-quality and compassionate care is consistently delivered
* Manage day-to-day clinical operations, including patient assignments, and case load oversight
* Perform initial patient assessments, including home safety evaluations, and develop individualized plans of care in collaboration with physicians and families
* Conduct supervisory home visits to ensure adherence to care plans, nursing standards, and regulatory guidelines
* Monitor clinical documentation for accuracy, timeliness, and compliance with state, federal, and accreditation standards (e.g., CMS)
* Participate in performance evaluations, corrective actions, and staff development plans for nursing personnel
* Provide clinical guidance and support to field staff for complex cases or emergency situations
* Collaborate with intake, scheduling, and administrative departments to support smooth patient transitions and service coordination
* Lead and/or participate in interdisciplinary case conferences and care team meetings
* Serve as a clinical liaison with physicians, specialists, schools, therapists, and external case managers
* Ensure proper implementation of infection control, medication safety, and emergency protocols in the home setting
* Assist in the recruitment, interviewing, onboarding, and orientation of new clinical staff
* Maintain current knowledge of pediatric conditions, treatments, and home care trends through continuing education
Qualifications:
* Registered Nurse (RN) with a valid state license (required)
* Minimum of 2 years nursing experience
* Prior experience in clinical supervision or nurse management a plus but not required!
* Strong leadership, communication, and organizational skills
* Ability to travel locally for home visits and supervisory duties
* Trach / vent experience or an interest in learning!
