The Interventionist is a critical member of the HBCI (Home Based Crisis Intervention) team. Primary Job Functions
As a member of the HBCI team, provides crisis intervention and intensive case management to youths aged 5 - 20 years to ensure safety and stabilization with the child/youth and caregivers/family in the home, school, and community environments.
Works collaboratively with the HBCI team to respond to referrals and ensure the referred youth meets the criteria for receiving HBCI services as defined in HBCI guidelines provided by the NYS Office of Mental Health.
Carries a caseload of up to 3 families providing 4-6 weeks of intensive intervention to include meetings with the child/youth and parents/guardians 3-5 days per week.
Implements individualized treatment interventions developed collaboratively with the family and based on identified clinical and family system needs.
Works with the team to plan and facilitate group therapy and psychoeducation for parents as well as youths.

We're looking for a dedicated Mental Health Technician to join our team and help us continue delivering the level of care our patients and families deserve. What You'll Do
* Coordination of Patient Activities: Assists in maintaining the flow of the activity program as set up by Activities Therapist, including maintaining outings, activity room.
* Patient Groups: Participates in unit groups as assigned, including co-leading group therapy sessions as assigned; completes required documentation in patient record. Participates actively in the development of new patient groups and milieu activities. Co-leads community meetings as assigned, and maintains accurate documentation; maintains posted unit schedule on board.
* Patient Care: Monitors and maintains patient vital signs, ADL skills, input and output logs under direction of charge nurse.
* Patient Monitoring: Monitors patients on constant and close observations; documents on appropriate flow sheet. Participates in keeping a safe environment conducive to maintaining a restful sleep environment. Conducts patient rounds including every 30 minute checks, constant and close observations, documents and complies with established unit safety guidelines. Provides supervision of patient in the kitchen area as required.
* Communication: Notifies charge nurse of patient behaviors in the milieu at the end of each shift and the patient's responses. Meets for 15 minutes with 2-4 assigned patients per shift, and documents interactions in the patient record, coordinating notes with the goals and objectives of the treatment plan.
* Admission and Discharge: Participates in and assists Registered Nurse with admission intake process by completing the initial screening questions, safety checks and orientation to the unit. Participates in patient discharge planning under the direction of charge nurse.
* Clerical Function: Assists in the maintaining the flow of documentation, ordering forms, list, materials and filing as required. Maintains daily cense sheets and reviews medical record data to be reviewed by treatment team.
* Deliver compassionate, patient-centered care in alignment with Saratoga Hospital values
* Collaborate with interdisciplinary teams to support health, healing, and service excellence
* Perform clinical responsibilities with a focus on safety, quality, and efficiency

The HBCI Program aims to serve children/youth ages 5 to 20 years at imminent risk of psychiatric hospitalization or admission to a group treatment program, or at risk for a rapid readmit to such settings.
Under the supervision of the HBCI Clinical Supervisor, the HBCI Interventionist responds to referrals and provides treatment to address the clinical and complex needs of the child/youth and their family/caregivers. In addition to providing direct crisis intervention and stabilization, the Interventionist provides case management services linking the family with additional short and long-term available services.
Essential Functions:
The major responsibilities of this position include but are not limited to the following:
Respond to each referral within 48-hours.
Meet with each youth and family/guardian to discuss program options, service provision, and alternative service options.
Conduct all admission paperwork and ensure all necessary consents/releases are signed.
Collaborate with already established providers such as care managers, therapists, and psychiatric providers.
Provide intensive crisis intervention and supportive counseling to the youth and family.
Participate in team meetings.
Work with community agencies, organizations, and systems for long-term planning and support of the youth and families served.
Make referrals to mental health services and other programs as needed.
Provide thorough documentation regarding all case related activities including but not limited to; face to face contacts, phone calls, case conferences, clinical interventions, noted incidents, etc.
Represent CHJC as a liaison with various private as well as public service providers and resource agencies.
Complete all CHJC and New York State mandated reports and forms in a timely fashion.
Continuously promotes the program and provides community education.
Participates in an after hours on-call rotation.

Ensures comfort and safety in the environment for individual patients, families, self and coworkers through good body mechanics when assisting patients with mobility and transportation, proper use of equipment, following policy for restraints and seclusion, and use of monitoring techniques.
Makes frequent rounds observing patients to anticipate possible patient needs.
Plans, initiates, and assists patients with unstructured activities to address leisure time needs and encourages patient participation in all activities.
Reports patient or unit problems to the appropriate RN or Mental Health Tech.
Assists with ECT treatments according to procedure including obtaining, set up and cleaning of equipment; providing support and assistance to physicians and nurses; transporting patients; and monitoring patient oral intake post ECT recovery.
Follows procedures in caring for the potentially violent patient and the patient in danger of hurting him/herself or others including monitoring all levels of observation, assisting with the initiation of seclusion and application of restraints, and caring for restrained and secluded patients.
Responds to Code Blue situations in all areas of the hospital as directed utilizing all CPI principles in verbal de-escalation, physical restraint (appropriate to age), 4 point restraint and maintaining a safe environment in all situations, under the clinical supervision of a CPI trained RN. Initiates and maintains appropriate and accurate documentation as required by UHS policy and procedure.
Escorts patients away from the hospital campus and works closely with the interdisciplinary staff in the planning of transfers.
Performs duties such as errands, covering telephones, and other limited tasks on the units.

The Academic Interventionist is responsible for working directly with students to provide tutoring, coaching, goal-setting, and academic case management to achieve measurable improvements in academic performance.
Duties/Responsibilities
Instructional Intervention
Develop academic lesson plans based on students' individual learning needs as required: intervention should be engaging as well as enable learning and recall of new knowledge strengthened through different exposures of "doing" (i.e., seeing, hearing, touching, etc.)
Use data to facilitate students' improvement and increase engagement as a result of their sustained participation in KIPP's blended learning programs
Provide students with daily feedback on mastery performance in character and academics

Document shift progress and care plan notes assessing progress toward achieving individualized care plan goals; document I & O's and ability to follow a continence plan; use of bladder scanner and Foley care
Receive specific training from OT, PT, SLP and Rec Therapy on all types of transfers, mobility activity and safe swallowing technique
Interact directly with patient during therapy sessions; employ Agitated Behavior Scale in combination with provider orders to deescalate patient behavior
Educate the patient's caregivers to make sure that they understand the patient care plan while hospitalized as well as the post-discharge support plan of the patient
