Perform assessment of goals and complete case management addendum within 48 hours of admission. Educate patient/family on rehabilitation and Case Manager role; establish communication plan. Schedule and facilitate family conferences as needed. Assist patient with timely procuring/planning of resources to avoid discharge delays or issues. Monitor compliance with regulations for orthotics and prosthetics ordering and payment. Make appropriate/timely referrals, including documentation to post discharge providers/physician Ensure accuracy of discharge and payor-related information in the patient record Participate in utilization review process: data collection, trend review, and resolution actions. Participate in case management on-call schedule as needed. Qualifications License or Certification: Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling). If licensure is required for one's discipline within the state, individual must hold an active license. Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position. CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position. Minimum Qualifications: For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree. For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred. 2 years of rehabilitation experience preferred. Primary Functional Responsibilities: Sustain the quality management system in partnership with Operations. Maintain systems to specification and standards (customer and 3rd party) relating to activities and products. Monitor and audit performance according to agreed standards and take necessary action to communicate according to performance levels. Communicate improvement and awareness of quality issues across assigned departments. Implement changes in standards (internally) and performance against standards. Perform against agreed targets within policies and standards. Complete raw material and finished goods audits and testing. Essential Qualifications and Skills: HS Diploma/GED required, Associates degree in technical field preferred or will consider commensurate experience. Ability to build positive partnerships and work collaboratively. Demonstrated problem-solving and root cause analysis skills. Ability to prioritize and multitask various and conflicting responsibilities in a high-energy production environment. Computer literacy, including working knowledge of the Microsoft Office Suite. Ability to use common Quality measurement tools such as vernier caliper, micrometer, dial gauges, thread, ring and plug gauges, profile projector, coatmeter, roughness tester, check sheet, cause and effect diagram, control chart, histogram, pareto chart, scatter diagram and flow chart. Preferred Qualifications and Skills: ASQ Certification: CQE, CQM, CQA, CQT, CRE preferred. Knowledge of Quality Systems (ISO-9001, ISO-14001, QS9000). Experience with ANSI A161.1. ISO-9000 Lead Auditor Certificate. Other Quality-related certificates. Six Sigma Orange Belt or comparable. The Caregiver will: * Assist with activities of daily living such as grooming, oral hygiene, toileting, bathing/showering, eating, etc. * Assist with personal and household chores, such as tidying up room, errands, delivering mail and/or newspapers. * Transport residents within the community to meals, enrichment activities, and other programs as needed. * Ensure resident care plans are reviewed and followed consistently. * Observe and monitor residents for behavioral changes and/or adverse reactions to medications or treatment plans. Communicate any changes to clinical staff immediately, as well as document changes thoroughly. * Respond to emergency calls, resident/visitor's requests and correct environment hazards in a timely, appropriate and safe manner. * Observe community policies and procedures regarding resident and team member safety, attendance, timeliness, house rules, teamwork, and resident service. * Maintain professionalism and resident confidentiality at all times. * Adhere to all Infection Prevention, OSHA, fire and safety regulations at all times, to ensure the safety and well-being of all community residents. * Perform all other duties as requested. Caregiver Educational Requirements: * Must possess a high school diploma or equivalent. * Caregiver, CNA or Home Health Aide certification is preferred, but not required. * Fulfill and maintain continuing education credits as required by state. * Must be willing to obtain and/or maintain current First Aid/CPR certification.
