The Registered Nurse Case Manager will work with our Enterprise Director of Nursing and Branch Manager to oversee the care provided to our clients. A typical day may include performing new client assessments, overseeing the care of existing clients (personal care and skilled), and care coordination with skilled service organizations and hospices. Compassion, patience and an outgoing personality are qualities that will build trust and a level of comfort with clients and their families. The Case Manager will work primarily (approximately 75%) in the field (local travel) and 25% of the time in the office. Benefits: 1:1 care with your clients NO WEEKENDS!!
