The Pharmacist, Regional Clinical Operations will be responsible for performing telephonic consultations with patients regarding the use of their specialty medication and management of their disease state. They will perform medication reconciliation, screen for potential drug interactions, and provide side effect management recommendations. Included in the core responsibilities of this role will be proficient and accurate in pharmacist final verification of a high prescription volume of orders prior to shipment. The Pharmacist, Regional Clinical Operations can expect to interpret prescribers' prescriptions, contact prescribers and/or patients to verify information on prescriptions, and expedite the processing of orders. This position will be expected to meet department production metrics and service levels, support quality standards, resolve patient issues, and vary work priorities and activities to accommodate business needs.

How do I make an impact on my team?
Verify prescriptions for pharmaceutical preparations and devices for patients at company facilities; maintain accurate and detailed records of all prescription transactions, and submit appropriate claims for third-party reimbursement; consult with physicians and nurses regarding pharmaceutical questions; advise physicians on formulary medications and cost-effective and innovative therapies; answer pharmaceutical questions from patients and staff; monitor storage, distribution, and use of pharmaceuticals.
Provide clear, detailed clinical documentation of patient consultations and interventions in pharmacy software in compliance with internal policies and procedures and external regulatory bodies (such as URAC) and/or EMR systems efficiently.
Provide oversight and quality assurance to pharmacy technicians.
Interpret and clinically evaluate prescriptions for specialty medications based on available information and resources. Contact providers and/or patients to verify information related to prescriptions such as drug strength, intended directions, and diagnosis to expedite processing of orders. Verify and confirm validity of controlled substances.
Contact providers for new prescriptions, consult with prescribers and nurses regarding clinical questions, and contact pharmacies for prescription transfers. Work with providers to convert prescriptions to generic, biosimilar, or preferred drugs whenever possible.
Verify prescription information entered in the system by data entry or order entry. Answer clinical questions from the pharmacy team.
Provide patient education through telephonic consultations for patients of the specialty pharmacy in accordance with Lumicera's Patient Management Program. The consultations will focus on effective assessment and communication of disease state management, side effect mitigation strategies, administration of medication, appropriate storage, and therapeutic appropriateness, while following a patient-centered care model. Answer clinical questions from patients and caregivers.
Document and report medication errors in accordance with company policies and manufacturer contracts. Facilitate error reduction and prevention methods based on errors and ISMP standards. Adhere to all Lumicera pharmacovigilance standards.
Contribute to the quality management program through daily operations and efforts toward sustaining key pharmacy performance metrics. Participation in quality improvement activities through routinely sharing ideas for improvements or implementing improvement opportunities with supervisors or quality team members.
Travel to designated locations to cover pharmacist absences due to vacation, leave, or staffing gaps. At times this will include limited lead time notice on travel requirements.

Qualifications

Graduate of ACPE Accreditation School of Pharmacy with a B.S. Pharmacy or PharmD degree
Current, unrestricted Pharmacist license is required in state where any on-site pharmacy work will be conducted. Must continually maintain education credits to maintain active licensure. Position Overview
Play a pivotal role in providing patient centered care by verifying patient specific prescriptions, validating compounded lots, and upholding compliance to USP and USP guidelines. Ensure appropriate dosing and verify compounded medications while upholding quality control and compliance according to Strive Pharmacy Standard Operating Procedures (SOPs).
Key Responsibilities
Follow proprietary formulations to compound commercially unavailable prescriptions into the appropriate dosage form
Performing calculations to ensure ingredients within a formula are measured and utilized properly
Inspecting lots and providing visual verification for the sterile laboratory through use of software and photos
Participate in regular audits and inspections
Ensure the compounding lab is cleaned and maintained in accordance with procedures
Maintenance, calibration and proper documentation of scales, electronic meters, etc.
Routinely checks for outdated or soon to expire chemicals
Coordinating with the Sterile Manager for personnel, compliance, and regulatory matters.
Developing and evaluating standard operating procedures for the compounding lab and formulations
Training and assessing pharmacy personnel
Complete compounding continuous education responsibilities
Understand and be familiar with cGMP guidance (21 CFR 211), 503B Guidance, USP 795, USP 800, USP 795, and cGMP FDA 503B Guidelines, and other documents as assigned
Required Qualifications
Minimum of Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD) Degree
Active MO Pharmacist license in good standing
One year of sterile compounding experience is preferred
Knowledge of USP 795, USP 800, USP 795 (cGMP FDA 503B Guidelines preferred)
Understanding of medical terminology and pharmacy calculations
Ability to work efficiently in a timely manner
Close attention to detail
Standards for accuracy and potency must be consistently met
Must be capable of working with small, delicate pieces of expensive equipment to complete appropriate dosage forms
Must be capable of donning and wearing all protective wear as required by procedure for each area. This includes, but not limited to: mask, gloves, lab coat, coveralls, hairnet, safety goggles and shoe covers
Must have knowledge of basic principles, theories, laws, policies and procedures that pertain to compounding and cGMP

Qualifications

Job Summary
Through effective leadership and coordination with a team of direct reports, the primary purpose of the Pharmacist II, Nuclear is to manage, facilitate, and oversee execution of day-to-day operations of the nuclear pharmacy. The Pharmacist II, Nuclear has completed all necessary training and is licensed to be a practicing nuclear pharmacist in their state of occupation. As such, this job has responsibility for optimizing pharmacy operations and procedures to ensure consistent and efficient delivery to control costs, minimize waste, maximize customer satisfaction and deliver positive patient outcomes while maintaining compliance with Federal and state regulatory agencies. Monday - Friday rotating shifts: 12am - 8am, 6:00am - 2pm, and 8am - 4pm

Authorized User: As defined in part by the Nuclear Regulatory Commission, an individual who has completed a minimum of 700 hours of training in a structured educational program consisting of both didactic training and supervised on the job training in radiation physics, instrumentation, radiation protection, chemistry, math and radiation biology. This is usually divided into two sections: 200 hours of formal course work and 500 hours of supervised experience. For current pharmacists, the requirements are the same plus a preceptor statement of training from an Authorized Nuclear Pharmacist.

The Manager Pharmacist In Charge is responsible for organization, coordination, direction, and evaluation of the provision of all pharmacy services, for supervising center staff and managing operating expenses of the pharmacy while providing high quality health care service.

Serve as liaison between specialty pharmacy, hospitals, home health agencies and other health professionals.

Work with the corporate office to maintain all state permits and PIC requirements and all regulatory changes.
Fiscal management of pharmacy expenses.
Initiate, monitor and report on QA&I activities.
Minimum Requirements:
Requires an RPh and a minimum of 5 years pharmacy experience and a minimum of 3 years management experience; or any combination of education and experience, which would provide an equivalent background.
Current active unrestricted state license to practice pharmacy as a Registered Pharmacist (RPh) in applicable state(s) required.
Preferred Skills, Capabilities, and Experiences:
Licensure in reciprocity states is required. (IL, NV, LA, OR, AL, AR, IN, KY, FL, SC)
PharmD preferred.
Prior experience with mail order and/or retail pharmacy compliance preferred.
Knowledge of DEA, FDA and state regulations pertaining to handling of controlled substances preferred.
Experience interpreting compliance requirements and regulations preferred.
Experience working with State Boards of Pharmacy regarding complaints, issue resolution, inspection and central fill requirements preferred.
