As a Substance Abuse Counselor you'll engage with your patients with individual, group, and/or family counseling. You'll work with a collaborative treatment team consisting of our Medical Director, Program Director, Treatment Services Coordinator, Counselors and Nurses to identify and meet the clinical needs of the patient and to assist in their overall well-being.

Essential Functions:
* Work with patients to complete intakes, admissions, discharges, and transfer paperwork.
* Document patient progress through counseling and interaction through groups.
* Complete patient psychosocial and an individualized treatment plan within the required time frame.
* Identify any clinical/case management needs and work to address those needs.
* Perform individual, group, and family counseling as required.
* Perform at least 20 hours of direct one-on-one patient contact per week through individual and/or group counseling sessions.
* Educate patients in all aspects of treatment, corresponding health issues and steps to recovery.
* Reviews Urine Drug Screens and initial patient photo identification.
* Always act in the best interests of the program and company; honors, supports, and protects the proprietary data and rights of the company.

THE ROLE
The Math Interventionist at Heartwood Community School provides specialized instruction to students who require additional support in mathematics. RESPONSIBILITIES
Deliver small-group or one-on-one math intervention sessions.
Identify students needing support through data analysis, teacher referrals, and assessments.

Monitor student progress and adjust interventions accordingly.
Conduct and interpret diagnostic assessments to determine students' skill levels.
Track student progress using formative and summative assessments.
Maintain detailed records of student performance and provide progress reports.
Use data to inform instruction and adjust teaching strategies.
Work closely with classroom teachers to align intervention strategies with the curriculum.
Provide professional development and support for teachers on effective math instruction.

Serve as a liaison between Horizons administration and families.
Work directly with the Horizons Parent Voice Committee to establish group guidelines for parent involvement in the program, develop a group structure, attend meetings and work with parents to establish goals for the group.
Plan and execute at least (4) special family engagement activities throughout the summer program including but not limited to Family Swim Night, Potluck Night, or new creative opportunities to share parenting resources with our families.
Participate in daily meals with the students, attend Family Night and Faculty Meetings, and other duties that may be required.
Attend paid pre-planning sessions prior to the start of the program.
Develop and nurture a safe, respectful, challenging and fun learning environment.
Assist in evaluating program effectiveness.
