Adhere to utilization, quality improvement, and corporate compliance policies and standards.
Provide services to clients as assigned by administrative/clinical supervisor.
Ensure and/or provide person-centered assessment and goal planning, of clients referred to the case manager from law enforcement. May provide screening, intakes, and assessment of clients utilizing required instruments and formats within appropriate timelines.
Follow-up with referrals from law enforcement to ensure clients have referrals to mental health services. Continue to follow up with clients who are not in outpatient services for a minimum of 60 days.
Co-respond to mental health crisis calls as appropriate.
Maintain documentation and data on clients referred to a case manager.
Observe clients in a variety of situations. Assess the mental status of clients and evaluate their current level of functioning.
Provide client crisis safety planning and incorporate clinician for additional assessment as deemed appropriate to protocol.
Conduct appropriate case management follow-up services to ensure continuity of care and client stabilization. Refer clients to internal and external supportive services such as vocational, medical, social services, housing authority, and residential and outpatient services.
Collaborate with other clinicians, law enforcement, external providers, and psychiatrists to arrange services for clients. Maintain close and cooperative relationships with community agencies and other public agencies in coordinating treatment and service recommendations.
Coordinate with clinical, residential, and hospital staff to identify client level of care needs and or anticipated needs upon discharge.
Maintain electronic health records in accordance with agency standards. Confirm coding of services rendered is ethically appropriate and results in successful billing. Document and maintain case management notes for clients who are in the hospital or in residential facilities. Documentation will include information regarding the anticipated discharge plan, timeframe, and final disposition.
Conduct intakes, ASIs, and answer questions regarding Volunteers of America services.
May learn, utilize, and support fidelity of any evidence-based practices within the assigned program to ensure effective interventions or treatment.
Provide communication with family members, guardians, and other involved parties, as permitted by law, and collaborate with them to develop suitable discharge plans and follow-up services for the client.
Advocate for clients in many different forums including federal, state, and local court systems, with family, and/or school system.
Attend staff meetings, clinical supervision, and other meetings as assigned.
Transport clients for necessary treatment services and/or community services.
May be called upon to occasionally make formal presentations or public speaking engagements representing VOA.

Here is What You Will Be Doing:
Greets, screens, and directs patients to exam rooms.
Obtains, verifies, and updates patient information and health history, collects patient information per provider guidelines, provides support services to patients and medical staff.
Reviews and prepares chart prior to patient's appointment to ensure that all requested lab work and diagnostic procedures have been completed, completes annual screenings on patients and documents in chart and refills patient medications as appropriate.
Assists patients with ambulation, transfers, assistive device instruction, body mechanic instruction, and activities of daily living.
Obtains patient vital signs, administers basic wound and first aid, and therapeutic modalities.
Administers subcutaneous injection, intramuscular injection, intradermal injection, long acting injections and medication administration as ordered (with the exception of narcotics).
Sends/receives patient medical records, obtains patient lab/X-ray reports, hospital or clinic notes and referral information.
Completes forms/requisitions as needed to ensure provider availability, schedules appointments, intake forms and verifies insurance coverage and pre-authorizations to ensure current and complete information in patient chart.
Collects urine and utilizes breathalyzer for Suboxone.
Assists running and monitoring Transcranial Magnetic Simulation machine with the assistance of the provider
Meets with psychiatrists to discuss follow up patient care needs.

This will be a part-time, position working outside school hours and on some weekends.

ESSENTIAL JOB FUNCTIONS:
· The Youth Participant will always be supervised and monitored by an FDA Commissioned Inspector during their work shift.
· The Youth Participant will receive training prior to conducting any purchase attempts.
· The Youth Participant will be driven to and from the tobacco retailer locations by the FDA Commissioned Inspector.
· The Youth Participant will attempt to purchase a tobacco product per the procedures provided within their training and the instructions provided by the FDA Commissioned Inspector.
· After the purchase attempt, the Youth Participant will leave the tobacco retailer with or without a purchased item.
· Purchased items will be immediately turned over to the FDA Commissioned Inspector.

Clinician will receive referrals for services. Conduct intake assessments. Complete required documentation and develop initial Treatment Plans. The Military and Family Life Counselor Mission Ready Reserve (MRR) position offers a unique opportunity to serve a vital role, in providing non-medical support, which includes counseling, consultation, and outreach, to our nation's military children and families. Non-medical counseling is preventive in nature and aims to address and provide solution focused approaches to issues before they become greater challenges.
