The DirectSupportProfessional (I) will work with each client, assessing their needs and meet those needs with the goal of fostering a positive, encouraging environment. This position is in a Day Service setting in which, the individuals we serve, will be given opportunities to volunteer, learn job ready skills, participate in group activities, join social clubs, as well as engage in community involvement. The ideal candidate will be willing to provide guidance, direction, encouragement, be patient, be a team player, as well as be hands-on to ensure that each individual reaches his/her full potential.
General Summary:
Ensuring all clients are treated with respect and dignity, recognizing individual needs and encouraging independence
Provide supervision and guidance for clients during skill building activities
Develop and implement individual treatment plans for clients
Assist with and work to strengthen the individuals' many daily living needs including but not limited to: nutrition, hygiene, exercise, communication and socialization skills
Develop a rapport with clients and cultivate a safe and supportive relationship
Update client case notes daily
Always ensure an appropriate staff-to-client ratio
Assist all participants in being actively involved in the daily activities to achieve greater independence using person-centered planning and self-determination.
Supervise lunch period.
Oversee arrivals and departures of participants.
Participates in staff training and attends staff meetings.
Always maintain the safety and security of the clients through line of sight supervision
Responsible in observing your client's negative behavior, the duration, the intensity, and the damage, factors that may alleviate or aggravate the behavior.
Ensures the safe loading/unloading and timely delivery of clients to their outings/appointments in compliance with all traffic laws, safety guidelines, current federal, state and local standards and regulations, as well as our established policies and procedures
Drives Company and/or personal vehicle in a safe and courteous manner.
Ensures all seat belts are fastened prior to moving vehicle.
Utilized Route Software and plans most efficient and effective route prior to transportation of clients to and from destination.
Other duties as assigned.
Qualifications: Education:
High school diploma/GED required
Minimum age of 19 years
Valid Driver's License

SPECIAL SKILLS, ABILITIES AND EXPERIENCE:
Three years' experience working with people with intellectual or other developmental disabilities preferred
Management of stressful situations responsibly and effectively provides input and assistance in planning and evaluating programs and clients' goals.
Maintenance and documentation of records of client's accidents/incidents as required.
Assistance of clients in being actively involved in the daily activities to achieve greater independence using person-centered planning and self-determination- ADL's- activities of daily living.
Strong interpersonal skills and passionate about helping those in need.
Relate positive image to clients, their families, support people and the community.
Employee must be able to read and write proficiently.
Employee must be able to successfully complete all required training in the required time
Ability to be flexible in a changing environment
Utilize verbal de-escalation skills during crisis intervention
Utilize approved behavior management techniques including use of de-escalation room and passive physical restraint when de-escalation and/or physical intervention is necessary for crisis management
Employee must be physically capable of transferring, assisting with transfers, assisting with lifting clients who require physical assistance
May be exposed to infectious/communicable diseases, blood borne pathogens and/or distraught individuals
Employee must be able to reach and bend as they assist clients with physical activities
Frequently required to stand and walk
Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects

II. This is a part-time/full-time, Para-professional level position. Specific Duties and Responsibilities:
1. The SCLS will teach, train, and assist individuals in developing the skills necessary to be as independent as possible when performing activities of daily living. These include but are not limited to:
a. Coordinating health and medical needs.
b. Training individuals to take care of their own health and medical needs.
c. Developing or maintaining skills allowing for better participation in the community.
d. Assist individuals served in becoming independent in accessing community services.
e. Assist the individuals with participation in at least one community activity each week if shift is during day/evening/weekend hours.
f. Assisting individuals in advocacy procedures to ensure his/her rights and to meet his/her needs, interests, and choices.

9. Work according to assigned work schedule. Duties may vary according to schedule, for instance some overnight shifts staff are expected to remain awake, and others are allowed to sleep as long as they remain available for emergencies.
10. Complete written and electronic documentation and record keeping according to agency requirements.
11. Must be able to work independently and ensure the welfare and safety of persons served in a residential setting.
12. Comply with state and federal regulations regarding HIPAA (Health Insurance Portability and Accountability Act).

14. Every employee/volunteer of Nishna Productions, Inc. is on a need-to-know basis regarding the Protected Health Information (PHI) of the individuals they serve. Therefore, a security clearance level is assigned for each job description and those security levels are allowed access only to the information required to complete the duties in each position.
• Level One allows the least access and is intended for truck drivers, maintenance staff, redemption center workers, and volunteers.
• Level Two is for direct care workers including floaters/substitutes, SCLS, Program Instructors, Day-Hab aids, Vocational Trainers, lead staff, and 1:1 staff.
• Level Three includes the office support staff.
• Level Four is for Team Leaders, supervisors, and Work Center Supervisors.
• Level Five is the security clearance given to Program Managers.
• Level Six allows access to all PHI and includes Directors/Administrators, Quality Assurance, and IT staff.
• Temporary clearance for PHI access can be given if staff require access to complete extra assigned duties (i.e. filling in for another department). Maintenance personnel are given permanent access to areas where PHI is stored but they may need to enter in order to complete their duties.

Individuals “must not have or be a carrier” of serious infectious or communicable disease, such as but not limited to, incurable, fatal, or debilitating disease, which cannot be eliminated or reduced by reasonable accommodation. Documents client progress to maintain a permanent record of client activity according to the agency's established methods and procedures. Participates in daily staff organizational meetings and treatment planning review meetings.
Contributes to the team's ongoing assessment of clients' mental illness symptoms and clients' response to treatment. Shares observations and concerns with the team for consultation and direction and takes appropriate action.
Utilizes recovery-based techniques to provide direct services to clients on an individual, group, and family basis in the office and in community settings to teach symptom-management.
Assists in skill development in the areas of personal hygiene and grooming tasks, nutrition education, meal planning, grocery shopping, food preparation, locating and maintaining a safe and affordable place to live, daily household maintenance, locating transportation services.
Provide transportation to assist clients with gaining access to resources.
Assist clients to obtain a primary care physician, dentist, and eye doctor.
Assist clients to plan and carry out socialization and leisure time activities.
Organizes and leads individual and group social and recreational activities.
Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
Is dependable and punctual regarding scheduling and attendance.
Abides by all specific program and Agency procedures, policies, and requirements.
Develops personal and program related skills through participation in internal and external training opportunities.
Performs other program related duties as assigned.
