Job DescriptionDescription:
The Care Coordinator I is responsible for providing care coordination services for clients in their
assigned group. The Care Coordinator I will assess clients with all care management needs and
address any potential barriers to care, to ensure timely diagnosis and treatment. Under direction of
the supervisor, the Care Coordinator I will maintain a clinical tracking system to ensure
appropriate follow-up, reporting and billing.
DUTIES AND RESPONSIBILITIES
Demonstrate proficiency and efficiency in processing referrals (specialty care and ancillary services), verifying insurances, and acquiring authorization for services if needed.
·Demonstrate proficiency in following up on referrals submitted and retrieving progress notes from specialist in order to close out referral loop and assisting with obtaining second referral if necessary.
·Demonstrate proficiency in documenting activities performed on clients in EHR and C3 database, as well as in looking up patient accounts and encounters in the EHR, EPM, and C3 database, using a variety of fields including but not limited to name, birth date, social security number, and account number.
·Documents and updates PHI log in EHR when referrals are sent/processed.
· Assist in updating to maintain patient demographics in EPM, EHR and C3 database.
· Assists providers with follow-up on referral as well as no-shows as directed by providers or contract staff. Assists with obtaining second referral if necessary
· Actively responds to phone calls from contractors and/or patients who may be in need of referrals or medical records for patient's continuum of care.
· Demonstrate a strong ability to scheduling, rescheduling, and cancelling patient appointments according to established protocols by service line (medical, obstetrics, dental, and behavioral health) and by provider.
Maintains faxes received in-patient EHR and acquires provider's signatures for faxes needing provider approval for services provided.
Flags charts for alerting staff regarding non-compliance services or needing updates to demographics.
Assist clinical teams in patient assessment via receipt of reports or review of charts that show abnormal labs/findings and notify clinicians in the event of an emergency.
Demonstrate proficiency in using great customer services skills/ proper phone etiquette when communicating with patients by phone, text messaging system, or by patient portal
· Completes daily tasks assigned by Director/Supervisor.
· Completes and runs rescreen report to bring clients back into service.
· Attend weekly meetings with department.

Daily vacuums all carpets.
Daily sweep and clear entrance walk ways.

Assists the dentist using four-handed-dentistry. Assist the dentist with patient management to include: instrument transfer, isolation, oral evacuation, retraction, and preparation of dental instruments and materials to include mixing filling and impression materials; charting examination and treatment results on patient's dental records.
Maintains autoclave, ultrasonic and other equipment as necessary. Maintains appropriate logs in accordance with maintenance.
Ensures monitoring logs for all infection control items are correctly completed. Maintains log of lab cases and reviews as needed.
Prepares, sterilizes, organizes and set-up dental operatory and instruments according to Dental Department OSHA manual guidelines.
At initial seating, ensures vital signs and medical history is reviewed and documented. Instructs the patient on oral hygiene, nutritional counseling and post-operative instructions, as needed. Ensures the patient receives verbal education and any post-operative homecare prior to dismissal.
Assists in maintaining adequate supply of inventory and follows protocol regarding ordering of supplies.
Flexibility to travel independently to other locations as needed.

Education: Requires High School Degree or GED plus graduate from accredited Dental Assisting Program. Must have Radiology, Infection Control and Jurisprudence certification. CPR Certified.
Experience: One year dental assisting experience in a dental office preferred.

! As the Breastfeeding Peer Counselor will have the opportunity to educate pregnant mothers and families about the benefits of breastfeeding. The Breastfeeding Peer Counselor will also assist mothers in successfully breastfeeding their infants.
DUTIES AND RESPONSIBILITIES
Teach classes on the advantages of breastfeeding and the management of normal breastfeeding experiences.
Provide information and promote breastfeeding among peers; addressing specific concerns of expectant mothers, and correcting misinformation which may prevent a pregnant woman from breastfeeding.
Provide hands-on assistance to mothers with breastfeeding problems which includes, latching and pumping breastmilk.
Counsel pregnant and breastfeeding mothers on a one-to-one basis.
Sanitize breastfeeding room and counseling offices after each use with Cavi-wipes.
Support women during a normal breastfeeding experiences, identify breastfeeding experiences that are not the norm, and make appropriate referrals.
Counsel by telephone on breastfeeding problems and make follow-up phone calls.
Distribute breast pumps to those WIC participants who demonstrate a need.
Provide support and information to breastfeeding mothers who may need advice to continue breastfeeding while working and/or attending school.
Make hospital and/or home visits when necessary.
Answer breastfeeding calls received on breastfeeding helpline for CentroMed WIC.
Be knowledgeable of the TXIN system; including ability to complete prenatal and breastfeeding questionnaires as well as the issuance and return of breast pumps. This also includes the ability to upload documents as needed.

As part of the Federal Navigator Grant and under the supervision of the Manager, the Navigator I, position involves meeting and interviewing Community Health Center patients and other community members to determine eligibility and facilitate enrollment in various health programs, including Medicaid, CHIP, Title V, Primary Health Care, Methodist Healthcare Ministries, and Qualified Health Plans (QHP) through the Federal Marketplace. The role requires maintaining current knowledge of patient eligibility status, enrollment protocols, and changes in program specifications. This role includes assisting patients in making informed decisions regarding their health plans and ensuring the security and confidentiality of consumer information.
The Navigator will be required to complete up to 30 hours of Navigator training through CMS and the HHSC Community Partner Program to obtain consumer assistance certification and will be required to recertify on an annual basis.
DUTIES AND RESPONSIBILITIES:
• Meeting, interviewing and evaluating patient's eligibility using the VeritySource Medicaid Eligibility System and provides application assistance for health programs like Medicaid, CHIP, and QHPs.
• Maintain up-to-date knowledge of eligibility, enrollment procedures, and program details for various health coverage programs.
• Implement outreach and in-reach strategies to promote health insurance availability, especially in under-served and under-represented communities.

The Customer Service Representative for the Call Center Department serves as a primary source of contact for patients at El Centro del Barrio clinics and as such is responsible for welcoming patients into the system and ensuring they have positive experiences. The CSR is responsible for scheduling, rescheduling, and cancelling appointments for all patients/families requesting health care services. The CSR will answer general questions asked by callers, or transfer callers to other parties if appropriate. The CSR will also make appointment reminder phone calls to patients as required.
DUTIES AND RESPONSIBILITIES:
Demonstrate ability to provide excellent customer service to all callers including, whenever possible, “first call resolution.”
Complete all calls as efficiently as possible, while still providing excellent customer service, to maximize the number of calls the Contact Center can handle and to provide a high service level to all callers.
Demonstrate proficiency in looking up patient accounts and encounters in the practice management system, using a variety of fields including but not limited to name, birth date, social security number, and account number.
Demonstrate proficiency in scheduling, rescheduling, and cancelling appointments in the practice management system based on patient or clinic needs.
Demonstrate a strong ability to schedule patient appointments according to established protocols by service line (medical, obstetrics, dental, and behavioral health) and by provider.
Answer telephones using script, identifying self and name of clinic.
Demonstrate a basic understanding of insurance (especially Medicaid and CHIP) and eligibility programs.
Make appointment reminder calls to patients (if an automated system is not making the calls), and follow up on calls where the automated system is unable to contact the patient.
Maintain excellent personal attire to include uniform and nametag.
Demonstrate excellent attendance and punctuality.
Demonstrate ability to meet department metrics.
Abide by policies and procedures dictated in the Employee Safety Handbook.

JOB SUMMARY:
We are searching for our next
SUPER STAR
! As our LVN you will be responsible to assess, observe, evaluate, and document intervention on the patients of this facility. To provide patients with teaching, counseling, support, and quality patient care. To work as a member of a team assuring the best possible care for all patients. The LVN must be familiar with community health resources, social, and welfare services.
DUTIES AND RESPONSIBILITIES:
Takes patient history, vital signs, symptoms and conditions and documents such in the medical record.
Checks patient's height and weight. Administers screening tests, evaluates and records results.
Administers patient immunizations following clinical protocols and provider's instructions.
Administers medications and treatments ordered by the provider.
Maintains lab duties to include, but not limited to: daily quality checks and documentation in log books, specimen processing including related result documentation and Provider notification of abnormals, daily cleaning and disinfection of lab and equipment in accordance with infection control guidelines, submits QC and patient testing logs monthly.
Checks the treatment room for properly functioning equipment and stocks supplies.
Provides individual or group health education to patients, including the distribution of educational materials as needed.
Reviews chart documentation for completeness according to established indicators.
Travels to other Centro Del Barrio sites in carrying out duties.
Abide by infection control policies and procedures as dictated in the Centro del Barrio Infection Control Manual.
Abide by policies and procedures on hazardous materials as dictated in the Employee Safety Manual.
Abide by all safety policies as dictated in the Employee Safety Manual.
Demonstrate competence with patients aged 0 and older.
Participate in the agency's Quality Improvement Program.
Performs other duties as required.

Requirements:
QUALIFICATIONS:
Education: Graduation from an accredited school of Vocational Nursing. Must be currently licensed with the Board of Vocational Nurse Examiners for the State of Texas. CPR Certification.
Experience: Experience from a clinic or hospital preferred.
Other: Bilingual/Bicultural. Spanish

Job DescriptionDescription:
• Provides general physical examinations, medical evaluations and screening to clinic patients.
• Direct patient care in accordance with CentroMed's scope of services
• Shares responsibility in joint practice collaboration with the VP & Chief Medical Officer in providing on-going supervision of patient treatment modalities, progress and compliance.
• Provides diagnostic, treatment, and preventive medicine as indicated by mid-level Delegation protocol and/or VP & Chief Medical Officer consultation, in accordance with Texas State Board of Medical Examiner's Standing Delegation Order 193.1-193.8.
• Uses appropriate clinical judgment in treatment of patients in emergency situations when the physician is not immediately available. Such treatment is followed by physician consultation and/or referral as soon as possible.
• Makes appropriate specialty referrals when necessary, seeking prior consultation with on-site physician if needed.
• Assists in maintaining medical inventory.
• Participates in the agency's Quality Improvement Program.
• Demonstrates competence with patients aged 0 and older.

Requirements:
Nurse Practitioner
RN licensed to practice in the State of Texas and graduated from
an accredited Family Nurse Practitioner Program.
ANA Certification.
Approval by the Texas State Board of Nurse Examiners to practice
in the State of Texas.
Physician Assistant
Graduate from an approved Certified Physician's Assistant
Program, and certified to practice in the State of Texas.
Registered in Texas (see Texas State Board of Medical Examiners
Physician's Assistant Chapter 185.3 Registry.
Experience
Family practice or community health setting
(preferred)
Knowledge of Hispanic cultural influences/practices and a general
understanding of the population being served.
