No regularly scheduled weekends or holidays!* Position Summary: The Registered Nurse (RN) partners with the Director of Nursing to oversee the care of the individuals ensuring the best practice and care of the individuals served. This role fosters a "One Team" approach, ensuring collaboration among medical, clinical, and operations teams to achieve the highest quality of care and support. Key responsibilities include travelling to homes, providing the highest quality of care to include medication management, medical treatment needs, education, and providing support to the Direct Service Providers. Supervisory Responsibilities: Supervises Licensed Practical Nurses and Certified Nursing Assistants as needed per market. Primary Responsibilities: * Administration of medications and vaccinations as requested by the physician or psychiatrist. * Under the direction of the DON, identifies, coordinates and monitors the health care of the individuals served, especially those with complex medical needs at assigned sites. * Administration of first aid as needed. * Provides care as ordered by physicians. * Diabetic foot assessments. * Assists with coordination of care and treatment for the individual served to ensure psychiatric, medical, dental, and optical appointments are scheduled as needed. * Communicates and coordinates and physical health concerns and follow up treatment with physicians and psychiatrists. * Performs medication administration and initial medical classroom training and ensure state requirements are being met. * Educate Direct Support Professionals (DSP) on the use of scales, blood pressure, oximeter, thermometer, stethoscope, glucometer, wheelchair, and any other durable medical equipment or any other need that is new to the home that is ordered by a physician. * *Participates in medical on call rotation if required.* * Assists with development and revisions of medical policies and procedures. * Responsible for triaging medical concerns. * Audit the medication room, EMAR and EHR programs following state requirements. Education and Qualifications: * Valid RN license with a minimum of two (2) years previous experience preferred. * Must pass criminal background check * Must possess a valid Driver's License. * Proficient in speaking, reading and writing the English language. * Approved by state, federal and government entities to work within BSLS programs. * Ability to use office equipment, including BSLS information technology systems. Travel: A significant amount of *local* travel is required to visit BSLS locations.

Essential Duties & Responsibilities: Provides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care Provides Skilled Nursing visits according to visit schedule and notifies agency of need to alter schedule Reports significant findings to patient's physician and Clinical Supervisor as they occur Submits completed skilled nursing visit notes per agency policy on designated days as requested by Clinical Supervisor Submits change orders per agency policy Provides assistance with activities of daily living (ADL's) which may include personal hygiene, changing of bed linens, dressing and ambulation and transfers as the patient need arises and/or as instructed Maintains open lines of communications to all members of the continuum of care team Attends staff meetings, team conferences and educational in-services per agency requirements Is responsible for obtaining information provided at skilled nurse in-services and meetings and demonstrates appropriate follow-up related to information given at meetings and in-services Participates in Process Improvement (PI) program by assisting with collection of data and serves on PI team upon request Follows agency policies and procedures Performs these and all other duties as assigned by the Administrator Maintains productivity level at 90% Participates in discharge planning process per Medicare Criteria and agency policy and procedure Responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care Other duties as assigned Qualifications Required Knowledge, Skills and Experience Current, unencumbered LPN license in the state of practice Must maintain a valid driver's license and good driving record The ability to make sound professional clinical judgment The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs Proficient clinical skills Excellent verbal and written communication skills Proficiency in personal computer use, including e-mail, clinical, word processing, spreadsheet and presentation software Active CPR Certification Preferred Knowledge, Skills and Experience One year of experience as a home care licensed practical nurse and is competent in performing home care comprehensive assessment.
