The RN - Transitional Care Coordinator is responsible for assessing patients that are medically complex and at high risk for readmission to ensure a successful transition from the hospital to community. The Transitional Care Coordinator RN is accountable to reduce the challenges that patients face post discharge and prevent hospital readmissions. What you will do: * Monitor and deliver care for patients in the post anesthesia recovery phase of care. * Monitor and assist with procedures, deliver care modalities including care of patients undergoing cardioversion, ECT, line insertion, complex dressing/wound care and care of the critically ill patients while in the PACU setting. * Nursing staff supports patient care in all phases including pre induction, perioperative, intraoperative and post anesthesia. * Collaborative team approach, coordinate and direct care with surgeons, anesthesiologists, clinical affiliates, and RNs. How you'll change lives As a Registered Nurse (RN) at US Renal Care, you'll be an integral part of a cross-functional team, working under the supervision of a Charge Nurse and assisting and supervising Patient Care Technicians (PCTs) to care for patients living with kidney disease.
