Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
Participate in planning for and the execution of patient discharge experience.

Qualifications
License or Certifications
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACMTM certification upon entry into this position OR within two years of entry into the position.
CCM or ACMTM certification required OR must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.

Participates in mandatory Drug and Alcohol training; complies with required and requested testing. Ensures vehicle is compliant with MN/DOT regulations regarding; fire extinguisher, spill kit, first aid kit, flashlight and securements in addition to the pre/post trip inspection. Is aware of safety equipment on bus and responsible to safely use as needed.

About the Role
As a
Partner in Wellbeing
, the Direct Care Specialist provides hands-on assistance to individuals with mental health conditions, helping them with daily living activities and promoting overall well-being. They work under the supervision of a mental health practitioner and/or professional offering support and encouragement to help clients achieve greater independence and community inclusion.
Essential Responsibilities
Provides services as outlined in the client's treatment plan. Assist with daily living activities.
Participate and/or lead activities, social skills training, and other interventions designed to promote client engagement.
Provides a supportive presence, helping clients manage stress, cope with difficult emotions, and build positive relationships.
Observe clients for changes in behavior, mood, or physical health and report any concerns to the appropriate staff.
Monitor medication administration and encourage medication compliance.
Create a safe and supportive environment by implementing and adhering to facility policies, procedures, and safety protocols.
Accurately documenting client care, observations, and progress in accordance with guidelines.
Follow the daily menu and put together meals during shifts worked during meal times.
Accompanies clients to scheduled appointments and transports clients as requested.
Performs additional duties as assigned or requested.

Required Education, Certification, and Experience
Must have a high school diploma or equivalent.
Must pass a criminal background check through MN DHS NetStudy.
Access to a personal vehicle and the ability and willingness to use the vehicle in carrying out the position's responsibilities if required.
Valid driver's license and insurable under the agency's automobile insurance policy if required.
Free from substance use problems for no less than 2 years.
Knowledge, Skills, Abilities, and Other Attributes
Basic knowledge of basic mental health dynamics regarding the diagnosis, assessment, and treatment of an array of mental health, substance health, and other behavioral issues.
Knowledge of word processing, electronic records, email, internet, scan, and fax.
Ability to handle confidential information responsibly.
Skill in adaptability and flexibility, teamwork, and interdisciplinary setting.
Knowledge of Microsoft Office Suite.
Ability to communicate clearly and effectively both orally and in writing.
Ability to maintain client confidentiality per the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

** The Phlebotomist collects (capillary and venipuncture) blood specimens from patients of all ages, assists patients with non-blood collections and is responsible for specimen integrity through accuracy of patient identification, specimen labelling, collection steps and specimen transport. This position performs occasional point-of-care patient testing such as glucose. The phlebotomist provides exceptional customer service.
Essential Job Functions:
Performs patient identification and specimen labeling according to standard operating procedures.
Understands the phlebotomy processes and techniques in order to obtain quality blood specimens to ensure that correct decisions are made regarding specimen collection.
Performs, interprets and reports point-of-care testing (e.g. glucose) and quality control.
Processes laboratory and patient reports according to standard operating procedure, e.g. printing, recording, distributing, faxing, filing, and retrieving.
Recognizes problems, takes appropriate action, documents problems and corrective action, and follows protocol to notify the Supervisor of problems in a timely manner so as not to jeopardize patient care.
Actively communicates throughout the shift and manages priority requests in a courteous and timely manner to ensure effective department workflow. Follows up with STAT, ASAP, and Pre-op draws to ensure turnaround times are met.
Performs other duties as assigned or needed to meet the needs of the department/organization.
