VDC serves Veterans of all ages and with all types of disabilities who are enrolled in the VA health care system and are in need of nursing home care but can remain at home with care from a VCA. The VDC program provides Veterans an opportunity to receive home and community-based services, supporting them to live independently in their homes. This is a consumer directed program. Job responsibilities will be determined by the need of the Veteran. This program enables the Veteran to recruit, hire, train, schedule and dismiss the Veteran Care Attendant. Position Overview:Key Responsibilities:
Provide patient-centered care for walk-in patients of all ages with acute and episodic conditions.
Collaborate with onsite staff and refer patients to appropriate specialists or emergency services as needed.
Document patient encounters thoroughly and accurately in the EPIC EMR.
Monday through Friday hours and participation in Saturday rotation; clinic is closed Sundays.
Qualifications:
Current NYS licensure as a Nurse Practitioner or Physician Assistant
Minimum 1-2 years of experience in walk-in, urgent care, emergency, or family medicine required
Strong diagnostic and clinical decision-making skills
Ability to work in a fast-paced environment
Commitment to high-quality, compassionate patient care
