Key Responsibilities:
Conduct comprehensive physical therapy evaluations in patients' homes.
Develop individualized treatment plans based on physician orders, assessment findings, and patient goals.
Deliver evidence-based therapeutic interventions to improve strength, balance, mobility, and functional independence.
Educate patients, caregivers, and families on home exercise programs, safety techniques, and assistive device use.
Monitor and document patient progress, response to treatment, and any changes in condition.
Communicate regularly with physicians, nurses, and other interdisciplinary team members to coordinate care.
Ensure compliance with agency policies, state practice acts, and federal regulations.
Maintain accurate and timely clinical documentation according to agency and payer requirements.
Provide input for discharge planning and recommend continuation of therapy or community resources as appropriate.
Participate in case conferences, quality improvement activities, and ongoing professional development.
Qualifications:
Licensed Physical Therapist (PT) in Michigan.
CPR certification
Minimum 1 years of clinical experience, preferably within home health in the last 5 years.
Valid driver's license, reliable transportation, and proof of insurance.
Strong clinical reasoning, communication, and time-management skills.
Compassionate, patient-centered approach with the ability to work independently.
