Key Responsibilities:
+ Oversee and ensure quality medical services delivered by hospice physicians across multiple locations.
+ Certify and recertify hospice eligibility, ensuring timely compliance with Medicare and other regulations.
+ Conduct and supervise face-to-face patient visits, including home consultations for complex cases.
+ Lead physician recruitment, training, supervision, and performance evaluation.
+ Provide 24/7/365 on-call physician coverage coordination with hospice physicians.
+ Maintain accurate, timely medical documentation and support regulatory reviews.
+ Participate actively in interdisciplinary group (IDG) meetings to coordinate patient care and medication management.
+ Monitor pharmacy utilization, ensuring appropriate medication prescribing and administration.
+ Collaborate with hospice leadership on policy development, compliance, and quality assessment and performance improvement (QAPI) initiatives.
+ Serve as a community liaison, enhancing hospice visibility and partnerships with healthcare providers.
+ Support educational programs for staff, physicians, and community partners.
About You
Qualifications:
+ Doctorate degree in allopathic or osteopathic medicine from an accredited medical school.
+ Board certification in a specialty recognized by ABMS or AOA-BOS, preferably primary care.
+ Current and unrestricted medical license in the practicing state.
+ Valid National Provider Identifier (NPI) and DEA registration for controlled substances.
+ Hospice and Palliative Medicine board certification and/or Hospice Medical Director Certification Board certification preferred.
+ Current Medicare Provider Enrollment (PECOS) and Medicaid certification if required.
+ Ability to travel 15-25% based on program needs.
+ CPR and TB clearance required.

* Take full ownership of meeting or exceeding weekly performance goals. Grow productivity levels on / ahead of pace with tenure-based company expectations
* Cultivate and manage strong, trust-based relationships with referral sources leveraging a personal, needs-based consultative sales approach
* Show genuine compassion and empathy to patients and families. Maintain resilience in the face of the significant emotional challenges inherent to working in hospice care
* Always prioritize the patient. Be ready / willing to work outside standard business hours, including evenings and weekends, to ensure timely support and service for patients in need

How you make a difference
The Mental Health Professional provides clinical services to individuals under the supervision of the Mental Health Coordinator, Clinical Supervisor or Mental Health Director. They conduct assessments, develop treatment plans, and provide evidenced-based psychotherapeutic interventions. Additionally, they provide mental health consultation and training to facility staff.
Key Responsibilities
* Provide clinical services to persons served in compliance with policies and relevant professional standards, including individual and group psychotherapy, crisis intervention, and diagnostic evaluations.
* Complete relevant clinical documentation, including clinical service reports and mental health treatment records, in a timely and professional manner.
* Provide consultation services to institutional staff, including screening committees and psychiatric staff, regarding the mental status and management of persons served.
* Participate in staff meetings, in-service training programs, and specialized clinical services and/or program development activities.
* Maintain accountability for services provided through participation in the Quality Improvement program and timely and accurate recording of activities.
Additional Details
Schedule for this position is every Friday and Saturday from 10:30pm-7am. This is a less than full-time position supporting hospice patients and their families in the Racine/Milwaukee areas. We are looking for an individual to assist with visits during the weekday hours.

As a Hospice Social Worker (MSW), You Will:
• Serve as the psychosocial support expert on the interdisciplinary care team
• Provide assessments and care planning based on the emotional, psychological, environmental, and cultural needs of patients and families
• Deliver counseling, crisis intervention, and practical resource support aligned with each patient's individualized plan of care
• Work closely with patients, families, physicians, nurses, chaplains, and other care team members to support the emotional and social well-being of the patient-family unit
• Educate families on hospice services, advance care planning, grief, and coping strategies
• Assist with long-term planning, community referrals, and resource navigation when needed
• Monitor and document psychosocial changes in the patient or family situation and recommend plan of care updates as appropriate
• Uphold ethical standards and professional judgment in end-of-life care
About You
Qualifications - What You'll Bring:
• Master's Degree in Social Work (MSW) from a CSWE-accredited program
• Current licensure as required by the state of employment
• Minimum of 1 year of experience as a Social Worker in a healthcare or hospice setting

How you make a difference
The Physician is responsible for assessing and treating patients' physical health including conducting exams and coordinating outside referrals. The role includes oversight of clinical care activities provided by ARNPs, documentation, and after-hours on-call responsibilities. This position performs minor procedures and collaborates with psychiatric and interdisciplinary staff to ensure integrated care. Physicians also educate patients and families, ensure compliance with safety procedures, and tailor treatment plans to the patient's age and development. Travel may be required depending on facility needs, and work is primarily in correctional or clinical settings.

* Assess patients' physical health through examinations and diagnostic procedures

How you make a difference
The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time.

* Maintains accurate and legible progress note documentation in SOAP format and adheres to formulary guidelines.

* Current CPR, negative TB screen and Hepatitis consent/declination
* Two years' experience as a Physical Therapist in an acute care or rehabilitation setting
Knowledge and Skills:
* Therapy skills as defined as generally accepted standards of practice
* Good interpersonal skills
* Knowledge of durable medical equipment
