For more information, please visit ********************* Program Overview: Intercept , developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children. This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program. Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills. Essential Duties and Responsibilities: The PRN Community-Based Paraprofessional: Coordinates transports and supervised visits and maintains contract-required documentation Teaches and demonstrates parenting skills Provides daily and weekly reports to the referral source and direct supervisor Maintains professional communication with referral sources Completes ongoing assessments as outlined by the referral source Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development and quarterly booster trainings to enhance clinical skills Completes accurate and timely documentation in an electronic medical record system (EMR) Performs other duties as assigned Additional Information: Schedule is flexible and non-traditional as it is based around the availability of the youth, adults, and families served. Ability to verbally communicate with residents, staff and general public in a manner that is understandable, must be honest, dependable and exhibit a warm, cheerful, caring manner. Must be able to safely perform the essential functions of the job with or without reasonable accommodation. EDUCATION and/or EXPERIENCE Current CNA Certificate in good standing on file with the Indiana Nursing Assistant Registry. One year experience in long-term care or other health care setting desired. PHYSICAL DEMANDS Physical Will need to continuously stand and walk, squat, bend to floor, reach, twist, sit and potentially remain in uncomfortable positions for extended periods of time. Must be able to lift, move and turn residents. Requires varying degrees of pushing, pulling and lifting depending on the weight of the resident, which is often times over 100 pounds. Must have eye/hand coordination sufficient to assist residents with ADL's, transfers, and ambulation. Must be able to clearly speak. Must deal calmly with multiple tasks, noises, and interruptions on a continuous basis. Sensory Must be able to hear normal sounds and voice patterns with constant background noise. Must be able to receive and follow verbal instructions. Must be able to respond to audible emergency signals, alarms, call lights and telephones. Must be able to smell smoke, spoiled food, incontinent residents, etc. Must be able to identify a resident within a reasonable distance (20 feet), focus and read thermometer, oxygen pressure gauge and similar equipment. Ability to identify colors for warning lights, signs and labels. Must be able to read regular/small size print. Cognitive Must be able to remember multiple assignments over an extended period. Must be able to remember specific requests, ideas and concepts and relate them as necessary. Must be able to concentrate on details during constant interruptions.
