Qualifications Candidates must possess a Bachelor's Degree from an accredited university or college with a major in; counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services field and have a minimum of one year of full time or equivalent experience working with adults with severe and persistent mental illness, OR Bachelor's Degree from an accredited university or college and three years, full time or equivalent, working with adults with persistent mental health diagnosis. Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population. Must have knowledge of available resources for adults with severe and persistent mental illness. Must have knowledge of the principles and practices of case management. Demonstrates proficiency in verbal and written skills. Must demonstrate proficient computer skills (40 wpm). Must be able to effectively utilize electronic health records. Management Expectations and Performance Standards Case Manager will maintain a caseload of 25+ clients, provide and document 138+ hours of billable services to clients per month, complete all opening paperwork for all new clients, review state outcomes quarterly, review FARS assessments and service plans bi-annually at their due date, complete initial leases and income certifications as well as their reviews annually, and complete all documentation on client services no later than 48 hours after the service has been provided. As a licensed clinician working for Health Connect America your NPI number will be used to bill for services performed by those being supervised by you. As a licensed clinician you will be responsible for ensuring that your credentials remain current, and you must comply with all training requirements. Effectively communicate with clients, colleagues, and other stakeholders to address inquiries, disseminate information, and resolve complaints or concerns. If providing services in NC, additional responsibilities include: Actively engage with individuals through assessment, coordination, health promotion, and transitional care, documenting assessments and coordinating with the care team and treatment teams.
