Assesses patient's needs for long term care including home and family situation, exploring alternatives to in home care arrangement for placement. Under supervision of the Director of Case Management, assists in the integration of Social Service functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies and healthcare facilities. Provides resources and services to assigned patients as appropriate. Ensures compliance with quality patient care and regulatory standards. The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations.
