The Forsyth County Department of Social Services is seeking a Sr. SocialWorker to work in the Adolescent Unit to provide state mandated services specifically to adolescents in the custody of FCDSS. Distinguishing Features * The Foster Care SocialWorker in this position has the responsibility of assisting families in developing permanent plans for their children who have been placed in the custody of the Forsyth County Department of Social Services.
* Permanent plans include reuniting children with the parent/caretaker from whom they were removed; custody/guardianship to an approved caretaker or relative; and adoption.
* The socialworker will be required to collaborate with the families to assess their needs; review and monitor their progress on goals; refer them to the appropriate community resources; complete required documentation and paperwork; and work within the guidelines of state mandated time frames.
* There will be regular court reviews on all cases and the socialworker will be responsible for writing court reports and presenting the information in court.
Minimum Education and Experience
Master's degree in social work from an accredited school of social work or;
Bachelor's degree in social work from an accredited school of social work and completion of the Child Welfare Collaborative or;
Bachelor's degree in social work from an accredited school of social work and one year of directly related experience or;
Master's degree in counseling in a human services field and one year of social work or counseling (Department of Social Services) or;
Bachelor's degree in a human services field from an accredited college or university and two years of directly related experience or;
Bachelor's degree from an accredited college or university and three years of directly related experience (Department of Social Services).
Valid driver's license required.
A valid driver's license is essential to the functions of this position, as regular travel outside of the office is fundamental to the daily work. Foster Care SocialWorkers must visit the children in their caseload in the child's home setting monthly, attend school-based meetings routinely, attend court hearings regularly, attend community-based meetings regularly, and respond to emergencies quickly.
Essential Duties and Responsibilities
Foster Care SocialWorkers must have a valid driver's license and the ability to work outside the office in places such as the client's home, court rooms, schools, etc. He/she must be able to make and report observations accurately without personal biases; and must be able to communicate effectively with persons from a range of socioeconomic backgrounds and mental capacities.
A Foster Care Senior SocialWorker should have excellent assessment skills, sound judgment, the ability to serve adolescents and families from all socioeconomic backgrounds with a range of mental capacities, without personal biases. Excellent written and verbal communication skills are also necessary for this position as well as a valid North Carolina driver's license.
The primary hours of work are from 8:00am until 5:00pm but flexibility in scheduling is needed to meet the needs of the families served and the mandated time frames.

CST is provided in a variety of locations. The team community-based mental health and substance abuse rehabilitative services and supports through a team approach necessary to assist adults in achieving rehabilitative and recovery goals.
Community Support Team (CST) provides direct support to adults with a Diagnostic and Statistical Manual of Mental Disorders Fifth Edition (DSM-5) diagnosis of mental illness, substance use, or comorbid disorder and who have complex and extensive treatment needs. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore a beneficiary's ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the beneficiary's community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal.
General Role Requirements: This is an intensive community-based rehabilitation team service that provides direct treatment and restorative interventions as well as case management. CST is designed to provide: a. symptom stability by reducing presenting psychiatric or substance use disorder symptoms; b. restorative interventions for development of interpersonal, community, coping and independent living skills; c. psychoeducation; d. first responder intervention to deescalate a crisis; and e. service coordination and ensure linkage to community services and resources.
This team service consists of a variety of interventions available 24-hours-a-day, 7-days-a-week, 365-days-a-year, and delivered by the CST staff, who maintain contact and intervene as one organizational unit. CST services are provided through a team approach, however discrete interventions may be delivered by any one or more team members if clinically indicated. Not all team members are required to provide direct intervention to each beneficiary on the caseload.
The Team Lead shall provide direct clinical interventions with each beneficiary.
Supervision Requirements: Clinical supervision for the CST Staff is provided by the licensed Team Lead who has the knowledge, skills, and abilities required by the population served. The licensed clinician facilitates a weekly face-to-face team meeting to ensure that the planned support interventions are provided; to allow the CST Staff to briefly discuss the status of all beneficiaries receiving services; problem-solve emerging issues; and plan approaches to intervene and prevent crises. The Team Lead monitors the delivery of CST to ensure the interventions are provided effectively to help the beneficiary restore community, daily living, personal, social and specific tenancy skills including obtaining and maintaining his or her own housing and develop natural supports, manage their illness, and reduce crises. Additional supervision or support may be provided as a group or with individual CST Staff as needed to address specific concerns or challenges.

Monitors utilization of service to ensure that it is effective, appropriate, and within the limits set forth in both rule, PCP, and the service authorization
Coordinates transition to another level/type of care for the person supported
Coordinates and oversees discharge planning process including development of a discharge plan initially upon admission and a discharge summary with follow-up resources at the conclusion of services

Under the direct supervision of the Director of School Behavioral Health and School Behavioral Health Regional Lead, this position works as a part of a multidisciplinary school-based treatment team to facilitate the provision of holistic, whole-person behavioral healthcare for all Kintegra Health patients. Provides behavioral health and substance abuse screening, brief intervention, education, and referral services for patients within a school-based setting. Provides ongoing consultation and treatment planning support for school administrators, socialworkers, and teaching personnel to meet the needs of individual clients and school programs. Will work with patients with psychosocial, behavioral health, chronic disease, behavior change, and/or substance use concerns. Is knowledgeable of and adheres to all policies, professional practice standards, philosophies and requirements for professional clinical practice.

School Behaviorist (LCSW, LCMHC or LMFT) Qualifications:
Education: Master's degree in Social Work or related field.

JOB SUMMARY

Main Responsibilities:
Provides psychotherapy services for residents to include individual, family, and group therapy.
Complete Bio-psychosocial evaluations, risk, and psychological assessments as appropriate.
Maintain all documentation according to facility guidelines including progress notes, case management, and individualized service plans.
Attends all treatment teams for assigned clients.
Completes psychosocial assessment within 7 days of admission.
Completes individualized service plans within 24 hours of a treatment team meeting.
Completes and updates all clinical notes within 24 hours after any therapy session, CFT, PCP, etc.
Available for Group Therapy, family involvement, and any other grouped area that may be specific toward the Clients treatment.
Attend and participate in monthly required court hearings.
Skills and Competencies:
Has program and managerial experience in a North Carolina public mental health setting which would provide knowledge of principles and practices of public mental health care, funding streams for such care, and related State and Federal requirements.
