Community Memorial Healthcare is hiring full-time master level behavioral health staff with excellent communication skills, clinical acumen, and passion for working with youth and their families. These positions play a key role in addressing behavioral health needs in primary care settings, improving access to services for children and their caregivers, and reducing health disparities among pediatric and vulnerable populations.
The Pediatric Behavioral Health Interventionist (BHI) supports the implementation and coordination of the Collaborative Care for Youth Innovation grant. This position operates within a Collaborative Care Model (CoCM) and Primary Care Behavioral Model (PCBH), integrating behavioral health into pediatric and young adult primary care settings. The staff provides screening, assessment, brief intervention, care coordination, and referral for patients with behavioral, developmental, or psychosocial needs. The role emphasizes early identification, family-centered support, and seamless transitions to appropriate care across child, adolescent, and young adult populations (ages 0-26 years).
The BHI staff is a core member of the collaborative care team. As a member of this team, you will work closely with primary care providers (PCPs), medical staff, psychiatric Interventionists, and other behavioral health professionals at Community Memorial Health to deliver comprehensive care that integrates mental and physical health. This position is based in Community Memorial Healthcare's outpatient primary care clinics and does not include hospital or inpatient responsibilities.
We are looking for clinicians who thrive in dynamic environments and demonstrate flexibility in meeting the needs of both patients and medical providers. MINIMUM QUALIFICATIONS:
Maintains an active License with the Board of Behavioral Sciences as a SocialWorker in the State of California and eligibility to serve as a field placement instructor and provide supervision of social work interns participating in the student training program. · Provide trauma-informed short-term counseling to individuals and families facing economic or housing instability.
· Conduct biopsychosocial assessments and service plans aligned with CSBG requirements.

Provides referral support to community resources and participates in inter-agency coordination of care.
Develops and maintains working relationships with community agencies, such as the Department of Social Services, psychiatric facilities, skilled nursing facilities, hospitals, social agencies, and may participate in hospital and/or SNF discharge planning.
Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and other people within the participants' support network (family, informal caregivers etc.).
Responds to, investigates, and reports complaints of elder abuse.
Maintains the confidentiality of all company procedures, results and information about participants, clients or families in conformance with Health Insurance Portability and Accountability (HIPAA) principles.
Active contributor and change agent to continuously achieve quality care, participant experience and compliance goals.
Proactively supports participant grievance and incident reporting processes.
Completes timely discharge planning.
Conducts home visits and travel to hospital facilities and/or other community agency, as needed.

Qualifications
Master's degree from a school of social work accredited by the Council on Social Work Education.
Minimum one year experience working with frail and elderly population. (3+ years social work experience preferred).
Experience conducting psychosocial assessments, care planning and case management skills required.
Demonstrated ability to work in a multicultural and multidisciplinary team environment.
Excellent oral and verbal communication and meeting facilitation skills
Previous experience assisting persons with behavioral health & substance abuse issues, preferred.
Bilingual ability preferred (Mandarin, Cantonese and Spanish)
Current CPR certification or the ability to obtain CPR certification is required.
Require personal transportation, current state issued driver's license, good driving record and auto insurance as required by law.

Meet productivity goal of a minimum of 65% of work time in direct service provision;
Assist in providing clinical information and support to the SRC treatment team;
Follow all COVID policies including cleaning, screening, and other protocols
Follow all company and government policies relevant to SRC operations;

QUALIFICATIONS:
Masters in field of social services (Social Work, Marriage &Family Therapy and Clinical Psych), with at least 1 years of experience working with marginalized populations
