ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned.

* Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements.
* Perform duties at all times within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations.
* Act as advocate on patient's behalf with the appropriate local, state, and federal agencies.
* Complete initial assessment of new patient psychosocial needs in adherence to state/CMS regulations and company policy.
* Complete KDQOL in accordance with company policy.
* Participate as a member of the Interdisciplinary team in all required patient assessment and care planning activities.
* Identify social agencies and other resources as appropriate to the needs of the patients, including but not limited to financial/funding sources.
* Be familiar with professional organizations related to kidney disease (i.e. National Kidney Foundation, American Kidney Foundation).
* Use personal protective equipment as necessary.
* Be familiar with all emergency operational procedures.
* Regular and reliable attendance is required for the job.
* Provide information and referrals as needed.
* Attend and participate in the monthly Quality Assessment and Performance Improvement (QAPI) activities.
* Coordinate development and maintenance of patient/family support groups.
* Maintain collaborative working relationship with Medical Director and physicians.
* Establish and maintain a positive relationship with area hospitals, agencies, vendors and the community.
* Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization.
* Participate in staff meetings as required. Attend in-service and continuing education offerings in compliance with company policy and procedure.
* Educate staff and provide inservice training in relation to patient psychosocial care.
* Lead staff in team concepts and promote a team effort.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Requirements include:
Must have Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure in applicable state is required unless employed in the state of AZ, PA or Guam. License must be maintained as current and in good standing. (See addendum for SocialWorker for minimum licensing requirements.)
Previous experience in providing social services to dialysis patients preferred.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Must have basic computer skills, including Microsoft Office (Word, Excel, Outlook); proficiency in all USRC clinical applications required within 90 days of hire.
Must meet any practice requirement(s) for the applicable state.

Responsibilities:
Under the supervision of the School Principal the SocialWorker is responsible for:
Individual and Group Counseling with students.
Complete annuals, quarterly reports, follow IEP goals and create goals.
Collaborate with teachers and interdisciplinary team embers to develop and implement Individualized
Education Program (IEP) goals.
Coordinates donations for families during the Holidays.
Conduct psychosocial assessments and develop treatment plans.
Provide counseling as listed in a child's IEP, maintain documentation as required by NYSED.
Assist families in understanding the developmental, and behavioral needs that are present in their children.
Offer crisis intervention and support to students in need.
Facilitate communication between school, families, and external agencies.

The Pediatric Hospice SocialWorker provides specialized psychosocial support to pediatric hospice patients and their families while also supporting the growth and expansion of the agency's pediatric hospice program. In addition to direct clinical services, this role serves as a liaison to community partners and referral sources, strengthening relationships with healthcare systems, schools, and community agencies to increase awareness, education, and access to pediatric hospice services.
This position requires strong pediatric social work expertise, exceptional communication skills, and a passion for advocacy and program development in pediatric end-of-life and supportive care. This is a full time, 40-hr per week, Monday - Friday position. Essential duties and responsibilities:
Psychosocial Care: Providing comprehensive psychosocial assessments for pediatric hospice patients and their families, addressing emotional, social, cultural, developmental, and practical needs; offering counseling, emotional support, and crisis intervention to patients, siblings, and caregivers throughout the course of illness and end-of-life; assisting families with advance care planning, goals -of-care discussions, and legacy-building activities as appropriate; providing grief, loss, and anticipatory bereavement support and collaborating with bereavement services for ongoing follow-up; participating as a corer member of the interdisciplinary hospice team, contributing psychosocial insight to individualized plans of care.
Pediatric Program Development & Expansion: Collaborating with leadership to support implementation and expansion of pediatric hospice and palliative services; identifying psychosocial needs, barriers, and service gaps impacting pediatric patients and families; assisting in the development of pediatric-specific resources, educational materials, and policies; participating in quality improvement initiatives related to pediatric hospice care and family support services.
Community Liaison & Referral Development: Serving as a liaison to healthcare systems, pediatric practices, schools, early intervention programs, and community organizations; building and maintaining relationships with referral sources including hospital socialworkers, case managers, pediatric specialists, and community providers; providing education and outreach on pediatric hospice eligibility, services, and psychosocial support offerings; representing the pediatric hospice program at community events, in-services, and professional meetings; collaborating with marketing, philanthropy, and leadership teams to support awareness and referral development efforts.
Documentation & Compliance: Maintaining timely, accurate clinical documentation in accordance with hospice regulations, agency policy, and payer requirements; ensuring compliance with CMS Hospice Conditions of Participation, state regulations, and accreditation standards; participating in interdisciplinary team meetings, care conferences, and quality review activities.
Qualifications
Master's degree in Social Work (MSW) from an accredited program
Current licensure or eligibility for licensure in the state of CT (LMSW, LCSW, or equivalent)
Minimum of 2 years of healthcare social work experience, and at least 1 year experience working with pediatric population.
Valid driver's license with reliable transportation and clean driving record with proof of auto insurance
Ability to work independently in home, school, and community settings
Preferred
Experience in pediatric hospice, palliative care, oncology, NICU/PICU, or complex chronic illness

· Assess and record medical, psychological, and social factors contributing to the individual's unique circumstances, including an assessment of strengths, needs, abilities, and preferences.
· Refer individuals to appropriate medical, psychiatric, housing and/or behavioral health services utilizing designated collaborating organizations (DCOs) whenever possible to ensure immediate admission into programs.
· Work closely with colleagues and community-based providers to ensure clients and their families receive access to timely, high-quality care and services, including follow-up to ensure connection. Serve as a client advocate in obtaining necessary services.
· Document all contacts; work collaboratively with the clinical team to assist in identifying and placing clients in appropriate aftercare services that offer interventions tailored to their unique needs and circumstances.

Duties and Responsibilities (these duties are a general summary and not all inclusive):
Assess, evaluate, conduct initial and ongoing case management of children/family services, and needs.
Develop, review, update and ensure implementation of strength-based service plans for each child in care or custody including risk assessment, safety plans and goals.
Complete all documentation in accordance with agency and regulatory requirements.
Attend home and foster care visits; transport children to health, social services or other agency-related appointments as required.
Maintain ongoing communication with DCF staff and other constituencies, initiate court action when necessary.
Empower families to make stable commitments to children by providing counseling and coordinating visits with biological parents and/or guardians and other relatives; develop a helping relationship and ensure needed supports and services are provided.
Attend weekly supervision, weekly staff meetings, in-service training, and team meetings.
Maintain a high degree of professionalism in the community, with clients, schools, courts and with referring agencies seeking to build and sustain positive relationships.

Minimum Entrance Requirements:
Applicants must have (A) a bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education or criminal justice or a relevant human services degree and (B) a current and valid Licensures as a Licensed Social Work Associate, Licensed SocialWorker, Licensed Certified SocialWorker or Licensed Independent Clinical SocialWorker issued by the Massachusetts Board of Registration*.
