Our Brands Responsibilities: The Family Centered Treatment Therapist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model. Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements. FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA). Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system. Deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments, with a family focus. Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule. Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards. Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision. Provide psychoeducation as indicated in the Service Plan. Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents. Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals. Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs. Coordinate and oversee the initial and ongoing assessment activities. Convene the Treatment Team for Service Plan. Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process. Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members. Qualifications: Minimum of a Master's Degree in human services or related field is required. Two years' experience working with youth or families. Additional state specific requirements include: NC, VA- QMHP or ability to register as a QMHP MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi. Perform Clinical Intake, provide diagnoses, collaboratively create treatment plans, provide ongoing care, and complete all processes for discharges. Maintain client collaboration with team members on client's care team - i.e. medication provider, PCP, OB-GYN, dietitian, other therapists, etc.; and client's family members. Participate in marketing and outreach efforts as needed, collaborating with Mamaya Health team for social media, website, and other activities to support client acquisition. Engage in timely communication with Mamaya Health staff and respond to email/chat within 2 business days, or faster depending on urgency of need. Regularly attend and participate in monthly small team meetings, quarterly all-staff meetings, monthly 1:1 meetings with Team Lead, and any additional supervisory meetings with Team Lead or Senior Clinical Leadership. Qualifications Master's degree or higher in social work, counseling, psychology, or a related mental health field. Independently licensed to practice counseling/mental health therapy (LPC-MHSP, LCSW) in the state of TN Direct clinical experience for 1 year is preferred. Ideal settings would include outpatient, intensive outpatient/partial hospitalization, or residential treatment settings. Experience working with kids and families preferred Experience working with women's and maternal mental health topics preferred Familiarity with evidence-based therapies, including CBT, DBT, and trauma-informed approaches. We are seeking a Licensed Clinical Social Worker (LCSW) to provide high-quality mental health services to individuals supported by our organization. The LCSW will collaborate with psychiatric and medical treatment teams to assess, diagnose, and develop treatment plans for individuals with mental health and behavioral needs. KEY RESPONSIBILITIES: Conduct clinical assessments and provide mental health treatment using evidence-based, strengths-based social work approaches. Evaluate clients social, family, and psychological backgrounds to inform treatment and care plans. Assist clients in responding constructively to treatment and in adjustment throughout and following discharge. Serve as a liaison between clients, families, psychiatric services, and community resources. Provide individual and group therapy in alignment with best practice standards and current clinical research. Interpret treatment plans for clients and families, helping reduce barriers to care and promote adherence. Participate as part of a multidisciplinary team in treatment planning, evaluation, and ongoing care coordination. Maintain accurate, timely documentation in compliance with regulatory and organizational requirements. Exhibit professional behaviors consistent with organizational values and ethics. Attend required training and maintain a safe work environment. Perform other duties as assigned by management. MINIMUM REQUIREMENTS: Masters degree in Social Work from an accredited program. Active LCSW license in good standing with the appropriate state board. MEDICAL & PHYSICAL REQUIREMENTS: Provide medical documentation as required by regulatory agencies. Full-time position; may require hours over 40 per week and on-call availability. Regularly stand/walk for prolonged periods; sit for short or long durations; occasionally stoop, kneel, crouch, or reach. Travel to meetings, trainings, and site visits, including overnight trips. Use all five senses effectively; vision must support reading, computer work, and operational tasks. Ability to lift up to 20 pounds occasionally. LICENSES/CERTIFICATES: Valid LCSW license. Provides diagnosis and treatment of mental or emotional disorders of individuals, couples, families, and groups in an outpatient setting. Role and Responsibilities • As a condition of employment, the healthcare provider will undertake to provide covered services, including emergency treatment, to a health center patient within the approved scope of project of Chota Community Health Services (CCHS), and to any individual who is not a patient of CCHS when asked, called upon, or undertakes, at or near any CCHS location and as a result of a non-CCHS patient's emergency situation, to temporarily treat or assist in treating that non-health center patient. • Collaborate with healthcare professionals to plan or provide treatment. Participate in Patient Centered Medical Home (PCMH) Care Team collaboration • Provides clinical consultation to primary care providers regarding behavioral health diagnoses • Provides assessment, diagnosis, and treatment of mental or emotional disorders of individuals, couples, families, and groups utilizing recognized modalities • Codes visits appropriately using the appropriate CPT and DSM5 codes • Consults other Behavioral Health staff, as appropriate, for patients with difficult or challenging situations • Acts as a resource for CCHS staff regarding mental health or Substance Use Disorder (SUD) questions or liaison to community organizations appropriate to patient's needs • Provide emotional and mental health counseling to groups or individuals • Build a strong rapport with patients and make accurate assessments of their problems • Keep detailed, concise records of each patient during the course of their treatment • Provide the most up-to-date and evidence-based treatment for CCHS patients with patients with mental health and substance use disorders • Functions under approved counseling protocols.
