Day to Day Responsibilities of this Position and Description of Project:
Managing a case load for healthcare members with LTSS (Long Term Support/Services) needs.
Monthly and quarterly member contact and will include 80% travel. Remote role. Will require a driver's license.
Member assessments and notes.
Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development.
Monitor delivery of services and follow-up with members, caregivers, or provider s through in person visits and telephonic contact
Authorize and coordinate referral for services.
Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care.
Assist in coordinating the development of informal or voluntary services to integrate into the member care plan Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-term care services!
Assist member with filing and resolving complaints and appeals.
Licensed Clinical Social Worker/Registered Nurse License and 2+ years of physical health care management experience, 4-6+ years of physical health care management experience preferred. Hospital CM, Home health, discharge planning, or long-term care experience preferred.
Licenses/Certifications: Valid driver's license

The Applied Learning Internship Program provides on-the-job experiential learning, exposure to numerous business leaders and functions, and formal educational opportunities to students within the Consumer-Packaged Goods and Beverage (CPGB) industry.
Interns work on function-specific business projects throughout the duration of the program with the support and guidance of their manager. Interns gain a broader understanding of the business and CPGB industry through guest speakers, customer field experiences, facility tours, and individual / team-based experiences (i.e., one-on-one meetings, team lunch, customer meetings, etc.). Job Summary
Serves as liaison between the Sterile Processing Department (SPD), distribution and the user departments inside and outside the healthcare facility. Manages daily functions in the SPD ensuring that established infection control practices are followed. Processes and exchanges items requiring sterilization from units, clinics, and facilities inside and out of the System.
Essential Functions
Supervises and coordinates the organization and cleanliness of the department.
Monitors schedule and works with the manager to adjust staffing according to work volume and complexity.
Functions as a liaison to clinical managers of user departments.
Maintains supply inventory, making revisions to par levels as needed.
Reviews instrument trays, medication management, and exchanges medical equipment, carts and procedure trays as applicable.
Assists in utilizing customer feedback to identify and implement programs for improving services.
Assists the OR service coordinators with the acquisition of instruments and trays.
Maintains documentation process for loaner instrumentation delivery and removal.
Assists with updating and maintaining manufactures recommendations for cleaning, packaging, sterilization and storage of instruments and equipment.
Physical Requirements
Must be able to stand and walk for long periods of time. Requires bending, stooping, reaching, and stretching. Must be able to lift up to 40 pounds and be able to push and pull heavy loads. Acuity in vision and hearing. Dexterity in both hands and fingers. Intact sense of smell and touch. Must be able to work under pressure and perform repetitive motion tasks.
Education, Experience and Certifications
High school diploma or GED required. Current SPD certification required. Minimum 3 years central or sterile processing experience required. Supervisory experience in a healthcare setting preferred. Ability to read, write and comprehend English preferred.
