Responsibilities: The Occupational Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in an independent living facility. Powerback Rehabilitation To You allows the Occupational Therapist to evaluate and treat in the most functional environment which is the patient's home. The Occupational Therapist organizes and conducts occupational therapy programs for the agency to facilitate rehabilitation. In addition, they direct patient participation in selected tasks to restore, reinforce, and enhance performance. The Occupational Therapist facilitates the learning of those skills and functions essential for adaptation and productivity, in order to diminish or correct pathology. This position provides a flexible schedule and work environment in various settings. 1. As necessary or applicable, supervision of Occupational Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.2. Responsible for adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings.3. Attends and contributes to patient care, staffing conferences and other related meetings.4. Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient.5. Consults with other members of the rehabilitation team, including the community physician, to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.6. Adapts program and/or plan of care according to the needs of individual patients.7. Promotes a safe environment and enforces the elimination of fire and safety hazards.8. Orders supplies and equipment as necessary.9. Cleans, maintains, identifies, and reports the need for repair of equipment as necessary.10. Honors the individual needs and preferences of residents and communicates well changes, requests and interests of residents with the Director of Life Enrichment. Assists Residents to and from life enrichment programs and outing as scheduled. Maintains good rapport with family members and encourages family support in programs. Assists residents during meals and snacks encouraging an atmosphere of socialization. Completes appropriate records indicating Resident's attendance and participation in individual and group activities in a timely basis. Provides input to the assessment and evaluation of Residents participation when requested. Communicates Resident's concerns and responses to interventions to immediate supervisor. Supports Residents individual needs, and preferences, customary routines, concerns and choices. Assist and works with individual volunteers and volunteer programs at the request of the Life enrichment Director. Assists residents with ADL needs that arise during planned activities such as toileting or eating/drinking. Participates in long term care survey process, regulatory, and accreditation. Requirements Qualifications and Skills High school diploma or general education degree (GED), or one to three months related experience and/or training or equivalent combination of education and experience preferred. Preferred Certified as a Nursing Assistant - 1-year experience preferred. If applicable: must possess a current chauffeur's license if operating transportation vehicles and possess a current Commercial Driver's License (CDL). Excellent communication and interpersonal skills. Enjoys socializing, has an outgoing and friendly personality Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
