The incumbent provides psychiatric and/or chemical dependency assessments, counseling, information, community resource referrals, and arrangements for continuing care and/or coordinated discharge planning. The social worker is responsible for advocating for the most appropriate and least restrictive services for the patient. Serves as an expert about, and as a liaison to, community resource programs and coordinates services between area providers. The Social Worker models a multidisciplinary approach to providing quality patient care and routinely involves Behavioral Health Nurse Practitioners, Psychiatrists, and all pertinent internal and external providers in developing treatment and discharge plans. While going to school the student works in a clinical capacity (if holds a current clinical licenser or certification) or non-clinical capacity at Northern Light Health. The Northern Light Maine Coast RN Sponsorship program consists of the following benefits: 1. Candidates accepted into the program will be awarded full tuition paid for up to a two-year accelerated nursing program or for the final two years of a nursing program of their choice. These years will be considered "sponsored years." 2. During the sponsored years, MCMH will pay for the employee to attend class and study, for up to 16 hours per week. 3. During the sponsored years, the employee will work as a patient experience aide or in certified nursing assistant role at Northern Light MCMH for 24 hours per week. Shares knowledge and serves as a resource in a team environment. * Attend class and lab related to school and study for said classes up to 16 hours/week; work the remainder of 24 hours when not in school or studying * Maintain commitment to work 40 hours/week when not in school Other Information: Must be currently enrolled in an accredited nursing program and be in Junior or Senior year if 4 year degree or the 2nd year for 2-year degree. Must be willing and able to provide copies of transcripts indicating completion levels and grades as well as 2 letters of recommendation (one clinical and one educational).
