About the Role:
We are hiring experienced NursePractitioners who are passionate about and who have recent experience in obesity medicine to provide longitudinal telehealth care for patients with obesity. In this role, you would collaborate with physicians and registered dietitians to deliver exceptional team-based care. This will be a full-time, remote position.
We seek applicants who:
Have a NursePractitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy
Are licensed to practice and prescribe legally, with preference for credentialing with insurance payers, in the state of New York
Have 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care, internal medicine, or similar setting
Have excellent interpersonal skills for patient and team interactions
Are committed to the highest quality of medical care and an evidence-based approach to obesity management
Have prior experience conducting telemedicine visits and who have a great "web-side" manner
Bilingual candidates with medical fluency in Spanish are highly preferred

Typical Appointments
In-person appointments consist of general medical evaluations in your practice. Evaluations are performed once per veteran and typically take 30 to 90 minutes. There are no ongoing treatment needs for these veterans; it is a one-time encounter.
We Schedule Around Your Availability.
Make your own schedule! Our current need is 3 days per week. We efficiently schedule a series of evaluation appointments that last no more than a total of eight hours on each of those days.

POSITION OVERVIEW
The NursePractitioner engages in the professional, organizational, and administrative activities to provide quality care to clients based upon their medical and psycho-social needs. The NursePractitioner along with the medical director, can develop, implement, and evaluate the medical practice delivery model in accordance with specific goals. The responsibilities of the NursePractitioner are leadership, direction and coordination in the provision of medical care and physician services supervised by the medical director and program director. KEY ESSENTIAL FUNCTIONS
Participating in planning, development, and organization of medical services provided.
Act as health coordinator to assure the provision of education, risk reduction, counseling and referral services to all patients regarding pregnancy, HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and all other communicable diseases. Also, training and education will be provided to all staff.
Meets with all scheduled patients efficiently and reviews scheduling with supervisor daily.
Reviews all medical policies related to patient care.
Conduct physical examinations of patients upon their admission and thereafter on an annual, regular and on an “as-needed-basis.”
Review the completion and appropriateness of all intake documentation, including patient health history (i.e., other medications and allergies), drug abuse history and previous treatment failures.
Review and approval of initial needs assessment and patient initial treatment plans and reviews.
Participates in preparation for regulatory agency site visits.
Knowledge of Federal, State and City regulatory requirements and ensures all aspects of clinical care conforms to these requirements.
Order, evaluate, adjust and terminate prescription of medication dosages using sound medical, ethical and responsible discretion and meets with the patient to discuss any adjustments in medication dosage.
Reviews all accident/incidents related to patient care and provides input into actions necessary to resolve identified problems. Implements corrective actions as necessary and appropriate.
Ensures physical participation in the Interdisciplinary Care Planning process.
Prescription and provision of treatment in accordance to patient's medical and mental condition and only upon their individual needs.
Determine physiological opiate dependency upon admission and patient eligibility for admission to program.
Participate in a weekly and on an “as-needed-basis” case conferencing sessions with MDT members to discuss preventative treatment measure to adverse changes as well as progress in patient's medical condition.
Assures medical staff compliance with all regulatory and licensure guidelines.
Assures medical staff competence with all applicable policies and procedures of the facility and initiates action in cases of non-compliance.
Participating in the comprehensive care planning, discharge planning and client and staff development by providing ongoing medical training and education to staff. Assume health coordinator role.
Providing medical assessment, consultation, treatment, and referrals that facilitate comprehensive and coordinated patient care.
Providing crisis intervention and consultation on medical/psychiatric emergencies.
Assessing client's need for hospitalization and consultation with client's Physician to include other care providers the patient is linked with and/or referred to.
Reviewing all patient medication and, if indicated, prescribes medication in consultation with Physician upon patient need.
Providing the Program Director with reports on recommendations concerning the medical services.
Provide general nursing services within scope of license and patient safety goals.
Complete medical records in accordance to organizational policy
Perform additional relevant duties as requested by supervisor/management.
REQUIREMENTS
Active and unrestricted New York State NursePractitioner license.
Active and unrestricted New York State Registered Nurse license.
Active and unrestricted NPI and DEA registration.
Masters Degree in Nursing/Related field completion of Accredited NP Program required.
Current Infection Control certification.

The Medical Cardiology unit is a 29-bed unit consisting of 1 private room, 13 semi-private rooms and one 4 bedroom. Patients are received from the Emergency Department, Cardiac Catheterization Lab, CCU, MICU, physician offices, and other facilities when inpatient admission is necessary.
The patient population served includes male and female, acute and chronically ill patients with a variety of cardiac and medical diagnoses. Most patients admitted to the unit require telemetry monitoring after receiving minimally-invasive and invasive cardiac procedures. The age range is from young adult (18 years) to geriatric (100+) years.

The NursePractitioner will work in a treatment team of psychiatrists, community mental health nurses, clinical social workers, licensed mental health counselors and credentialed alcoholism and substance abuse counselors. The NursePractitioner conducts initial patient evaluations and periodic reviews; provides addiction treatment and medication management to clients and monitors participant progress towards the attainment of individual goals; attends and participates in weekly supervision, weekly staff meetings and periodic in-service trainings and may present issues/cases for review. The practice utilizes the eCR TenEleven electronic medical record. At Saratoga County Mental Health and Addiction Services, our full time providers work a 35-hour work week. Outpatient County Addiction Medicine Opportunity - Part Time, Two Days Per Week

Saratoga Hospital seeks an experienced Psychiatric Mental Health NursePractitioner (PMHNP) or Family NursePractitioner (FNP) for Saratoga County Mental Health and Addiction Services - to be a part of a vibrant, creative team. Work as a member of Saratoga Hospital Medical Group - Behavioral Health in this community mental health setting.
The NursePractitioner will work in a treatment team of psychiatrists, community mental health nurses, clinical social workers, licensed mental health counselors and credentialed alcoholism and substance abuse counselors. The NursePractitioner conducts initial patient evaluations and periodic reviews; provides addiction treatment and medication management to clients and monitors participant progress towards the attainment of individual goals; attends and participates in weekly supervision, weekly staff meetings and periodic in-service trainings and may present issues/cases for review. The practice utilizes the eCR TenEleven electronic medical record.
At Saratoga County Mental Health and Addiction Services, our full time providers work a 35-hour work week. This will be 14 hours weekly, 28 biweekly.
There is some flexibility with hours. The office hours are 9:00 a.m. to 5:00 p.m. Mondays and Fridays, with a one-hour lunch break, and the practice is open until 8 on Tuesdays and 7:30 on Wednesdays and Thursdays. Saratoga County Mental Health Clinic and Addiction Services is located in a bright, spacious building renovated in 2013, within walking distance to downtown Saratoga and Saratoga Spa State Park.
Job functions include:
Patient assessment and treatment: This provider will meet with patients of our outpatient addiction treatment program (2 days per week) who have expressed a need for addiction services. This provider will gather relevant history, assess symptoms and prescribe medications as appropriate. This provider will have discretion as to how often to follow up with patients and will be allotted ample time with, generally one hour for assessments and 30 minutes for follow-up sessions.
Medication Assisted Treatment: This provider will assess patients who struggle with relapse, urges and cravings and prescribe, when deemed appropriate, certain medications to support addiction recovery. These include Buprenorphine, Naltrexone, Vivitrol and potentially others. This provider may be asked to perform injections of Vivitrol when prescribed or to perform any other injections that emerge in addictions treatment. This provider will work with the Program Coordinator and Medical Director to ensure that any controlled substances are prescribed carefully with special attention to the characteristics of the population and the potential to abuse or divert medication.
Health Assessments: This provider will work with the program coordinator and medical director to ensure that regulatory requirements are met regarding assessing patient health status, referring to healthcare services and encouraging patient follow through. In addition, this provider will act as the Health Coordinator for Addiction Services. This will include face to face, noninvasive meetings with individual patients to assess their health status. Beyond regulations, this provider will have the opportunity to craft a plan for encouraging the good health of our patients, potentially including individual or group work around health topics.
Multidisciplinary Team: This provider will participate in team meetings, collaborating with various other providers to ensure quality, comprehensive care. This provider will receive clinical supervision from a psychiatrist with decades of experience in psychiatry and addiction medicine. This provider will receive assistance from registered nursing staff with regard to coordinating patient care, managing medication refills and prior authorizations, and administering injectable medications.
Qualifications: This provider must be a licensed NursePractitioner in NYS by the time of credentialing, with experience in the assessment of psychiatric disorders and the provision of psychiatric treatment.
A NursePractitioner with fewer than 3600 hours of experience will be supervised by a Collaborating Physician, in this case, the Medical Director of the Program. Supervision will be provided at least monthly, and as often as weekly. More frequent consultations will be available as needed.
For Psychiatric Mental Health NursePractitioners, participation is expected, on a pro-rated level, dependent on FTE status, within our reasonable 1:10 (varies) hospital inpatient call schedule (paid) covering weeknights (by phone) and weekends. Weekend coverage includes rounding 8 - 5 p.m. on the mental health unit and performing new consultations. This would not apply to a Family NursePractitioner specializing in Addiction Medicine.
NursePractitioners and psychiatrists at Saratoga County Mental Health and Addiction Services are part of a larger collegial team that includes hospital providers; the Saratoga Community Health Center; Saratoga Hospital Medical Group Primary Care - Wilton.
